A Quantitative Analysis Examining Patients' Choice of Active Surveillance or Surgery for Managing Low-Risk Papillary Thyroid Cancer

Summary: This study aimed to understand the treatment preferences of patients with low-risk papillary thyroid cancer. The study found that approximately three-quarters of the patients chose active surveillance (AS) over surgery. Factors associated with choosing AS included older age, lower education level, and having a surgeon outside the study institution. Patients' concerns about thyroid hormone replacement and disease progression, as well as their level of active coping, were also associated with the decision.

Long-term outcomes of radiofrequency ablation for unifocal low-risk papillary thyroid microcarcinoma: a large cohort study of 414 patients

Summary: The management of patients with low-risk papillary thyroid microcarcinoma (PTMC; T1aN0M0) has become an important clinical issue, with the adoption of active surveillance (AS) to avoid overtreatment gradually increasing worldwide. However, not all PTMCs are suitable for AS, and the Japan Association of Endocrine Surgery has produced consensus statements regarding indications and strategies for AS in adult patients diagnosed with low-risk PTMC. These statements provide guidelines for AS and recommend immediate surgery for certain high-risk features in PTMC patients.

Contrast-enhanced ultrasound: a valuable modality for extracapsular extension assessment in papillary thyroid cancer

Summary: CEUS is a more valuable and non-invasive imaging modality compared to conventional US in detecting extracapsular extension in papillary thyroid carcinoma. Single capsular invasion was a poor predictor of cervical lymph node metastasis, while extracapsular extension assessments were clinically significant for predicting cervical lymph node metastasis. Among the thyroid papillary carcinoma nodules with differential enhancement, hyper-enhanced nodules had the highest incidence of single capsule invasion (41.9%), while hypo-enhanced ones had a higher incidence of ECE (47.4%).

Image-Guided Thermal Ablation as an Alternative to Surgery for Papillary Thyroid Microcarcinoma: Preliminary Results of an Italian Experience

Efforts to improve cancer prevention measures and provision of cancer care in transitioning countries will be crucial for global cancer control.
