Novel machine learning algorithm can identify patients at risk of poor overall survival following curative resection for colorectal liver metastases

Iakovos Amygdalos et al.

Summary: Machine learning algorithm can help identify high-risk patients with colorectal liver metastases (CRLM) and assess their overall survival (OS). Predictive factors based on clinicopathological characteristics include preoperative serum carcinoembryonic antigen (CEA) level, age, diameter of the largest metastasis, number of metastases, body mass index, and primary tumor grading. The application of this model can guide patient follow-up and treatment strategies.

Body composition and the skeletal muscle compartment in liver transplantation: Turning challenges into opportunities

Sophie Reichelt et al.

Summary: Frailty, nutritional status, and body composition are crucial factors in liver transplantation, with recent guidelines emphasizing their clinical importance. While previous focus was on quantitative changes in skeletal muscle mass, recent studies highlight qualitative alterations like myosteatosis and sarcopenic obesity as emerging risk factors for poor outcomes. The review discusses the role of body composition, particularly skeletal muscle, in liver transplantation, covering assessment modalities, diagnostic challenges, prognostic significance, and ways to incorporate body composition parameters into clinical decision making, as well as highlighting novel trends and challenges in therapeutic targeting of body composition and skeletal muscle compartment.

Cardiovascular Evaluation of Liver Transplant Patients by Using Coronary Calcium Scoring in ECG-Synchronized Computed Tomographic Scans

Anna Bettina Roehl et al.

Summary: In patients awaiting orthotopic liver transplantation, combining Lee's revised cardiac index (RCRI) and coronary artery calcium (CAC) scoring showed better diagnostic accuracy and prognostic impact for predicting cardiovascular events compared to standard non-invasive stress tests. The cutoff value of CAC > 0 by non-gated CTs combined with RCRI & GE; 2 is highly sensitive for identifying patients at risk for cardiovascular events in this population.

Prognostic role of the Donor Risk Index, the Eurotransplant Donor Risk Index, and the Balance of Risk score on graft loss after liver transplantation

Vladimir J. Lozanovski et al.

Summary: This study aimed to identify cutoff values for donor risk index (DRI), Eurotransplant (ET)-DRI, and balance of risk (BAR) scores that predict liver graft loss risk. DRI >= 1.4 and ET-DRI >= 1.4 were associated with the highest risk of graft loss at all time points, while BAR > 18 was associated with the highest risk at 3 months and 1 year. However, the sensitivity and specificity of these cutoff values were found to be low for predicting graft loss.

An explainable supervised machine learning predictor of acute kidney injury after adult deceased donor liver transplantation

Yihan Zhang et al.

Summary: An early prediction model for post-liver transplantation AKI was developed using supervised machine learning, with the GBM model showing the best performance in both internal and external validation sets. The SHAP method identified high preoperative indirect bilirubin, low intraoperative urine output, long anesthesia time, low preoperative platelets, and graft steatosis as the top 5 important variables influencing the diagnosis of post-LT AKI. This GBM-based predictor provides a highly interoperable tool for decision-making after liver transplantation.

Summary: Strategies to reduce the incidence of early allograft dysfunction (EAD) are needed to control costs in liver transplantation, as our study found that EAD leads to significantly higher hospital costs primarily attributed to recipient health status.
