Targeted Prostate Biopsy: Umbra, Penumbra, and Value of Perilesional Sampling

Summary: This study explores the utility of perilesional biopsies in detecting clinically significant prostate cancer. The results demonstrate that biopsies taken from a 10-mm radius outside MRI lesions contain most cores of cancer that are not present within the lesion, suggesting that sampling both within and around lesions increases sensitivity for cancer detection.

Why Does Magnetic Resonance Imaging-Targeted Biopsy Miss Clinically Significant Cancer?

Summary: Multiple studies have shown that MRI-targeted biopsy is better at detecting clinically significant cancer compared to systematic biopsy, but some clinically significant cancers can only be detected by systematic biopsy. This retrospective analysis aimed to determine the reasons why MRI-targeted biopsy missed clinically significant cancer that was subsequently detected by systematic biopsy. The analysis found that most misses were due to errors in lesion targeting, followed by MRI-invisible lesions and lesions missed by radiologists. Lower PI-RADS scores were associated with missed clinically significant cancer on MRI-targeted biopsy.

Prospective analysis of pain expectancy and experience during MR-fusion prostate biopsy: does reality match patients' expectancy?

Summary: This study investigated pain levels during MR-TB in 108 men with suspected prostate cancer. The results showed low overall pain levels, but significant correlations with anxiety, stress, and pain expectancy. Expectation and actual pain levels during sampling procedures differed significantly, with prostate volume and expected pain during rectal manipulation being independent predictors of increased pain.
