Summary: In this study, we investigated the changes in serum inflammatory mediators to identify potential biomarkers associated with delirium in older adult surgical patients. The levels of brain-derived phosphorylated neurofilament heavy subunit remained elevated throughout the perioperative period in patients with delirium. Additionally, the level of plasminogen activator inhibitor-1 was found to accurately discriminate between patients with and without clinically diagnosed delirium.
