Common incidental findings on sacroiliac joint MRI: Added value of MRI-based synthetic CT

Lieve Morbee et al.

Summary: The purpose of this study was to determine the prevalence of incidental findings on sacroiliac joint MRI and the added value of MRI-based synthetic CT for detecting and evaluating these findings. The results showed that incidental findings were more common than sacroiliitis on sacroiliac joint MRI, and more than half of these findings were only visible on synthetic CT.

What amount of structural damage defines sacroiliitis: a CT study

Kay Geert A. Hermann et al.

Summary: This study proposes a data-driven definition for structural changes in the sacroiliac (SI) joints in the context of axial spondyloarthritis (axSpA) imaging. Evaluation of 546 individuals with SI joint CTs showed that ankylosis and/or erosions of the middle and dorsal joint portions had the best diagnostic performance. Sclerosis and ventrally located erosions had lower specificity on SI joint CT imaging.

Choose wisely: imaging for diagnosis of axial spondyloarthritis

Torsten Diekhoff et al.

Summary: The study found that compared to CT and MRI, X-ray had lower sensitivity and inferior specificity for axSpA. However, CT showed the best inter-rater reliability, highlighting the importance of structural lesions for the differential diagnosis in axSpA.

Differences in topographical location of sacroiliac joint MRI lesions in patients with early axial spondyloarthritis and mechanical back pain

Rosa Marie Kiil et al.

Summary: Early diagnostics of axial spondyloarthritis (axSpA) remains a challenge, with traditional imaging techniques being used. By introducing a two-plane assessment system, differences in SIJ MRI changes between early axSpA and mechanical back pain (MBP) patients were analyzed, showing higher BME occurrence in axSpA patients and in the sacrum region for MBP patients. The study findings can help differentiate axSpA patients from other back pain conditions during early diagnosis.

Data-driven definitions for active and structural MRI lesions in the sacroiliac joint in spondyloarthritis and their predictive utility

Walter P. Maksymowych et al.

Summary: This study aimed to determine quantitative cut-offs for SI joint MRI lesions typical of inflammatory and structural lesions in axSpA, which can predict clinical diagnosis. The results showed that cut-offs for MRI findings specific to axSpA had PPVs greater than or equal to 95% for clinical diagnosis. Thus, the proposed cut-offs can be used for disease classification and clinical research to improve the diagnostic accuracy for axSpA.

Diagnostic delay in axial spondyloarthritis: a systematic review and meta-analysis

Sizheng Steven Zhao et al.

Summary: This study found that there is a significant global variation in delay to diagnosis for axial SpA patients, with longer delays reported in high-income countries compared to middle-income countries. Patient factors like lower education levels, younger age at symptom onset, and absence of extra-articular manifestations are associated with longer delays. In contrast, the diagnostic delay for PsA patients is significantly shorter.
