Biochemical Markers of Musculoskeletal Health and Aging to be Assessed in Clinical Trials of Drugs Aiming at the Treatment of Sarcopenia: Consensus Paper from an Expert Group Meeting Organized by the European Society for Clinical and Economic Aspects of Osteoporosis, Osteoarthritis and Musculoskeletal Diseases (ESCEO) and the Centre Academique de Recherche et d'Experimentation en Sante (CARES SPRL), Under the Auspices of the World Health Organization Collaborating Center for the Epidemiology of Musculoskeletal Conditions and Aging

Aurelie Ladang et al.

Summary: Biochemical markers in clinical trials provide useful information on drug's mode of action, therapeutic response, and side effect monitoring. Identifying biomarkers for sarcopenia management is urgently needed for future clinical practice. A consensus paper from a working group of the ESCEO proposed a list of biochemical markers for musculoskeletal health and aging in Phase II and III clinical trials for sarcopenia treatment, and suggested classifying them into two series based on their evaluation of musculoskeletal status and causal factors.

Development of multiple biomarker panels for prediction of sarcopenia in community-dwelling older adults

Hyung Eun Shin et al.

Summary: This study aimed to develop multiple biomarker panels to predict sarcopenia in older adults and explore its association with sarcopenia incidence. By analyzing data from 1,021 older adults, 8 optimal biomarkers were selected to develop a multi-biomarker risk score, which showed better discrimination for sarcopenia than single biomarkers according to receiver operating characteristic analysis. During a two-year follow-up, the incidence of sarcopenia was found to be positively associated with the continuous multi-biomarker risk score. In conclusion, the multi-biomarker risk score can effectively predict sarcopenia and its incidence in older adults.

Assessment of sarcopenia and malnutrition using estimated GFR ratio (eGFRcys/eGFR) in hospitalised adult patients

Sadao Yoshida et al.

Summary: This study aimed to investigate the accuracy and usefulness of the eGFR ratio as an alternative method for evaluating muscle mass. The results showed a moderate correlation between the eGFR ratio and muscle mass. The calculated SMI based on the eGFR ratio accurately diagnosed sarcopenia and malnutrition.

Estimation of Muscle Mass Using Creatinine/Cystatin C Ratio in Japanese Community-Dwelling Older People

Hiroshi Kusunoki et al.

Summary: This study developed a prediction equation to estimate skeletal muscle mass based on the creatinine/cystatin C ratio. The equation showed strong correlations between the predicted muscle mass and bioimpedance analysis-measured muscle mass. The equation also had high accuracy in identifying low muscle mass. This new prediction equation could be a useful tool for diagnosing sarcopenia.

Summary: This study found that IMCL content is negatively correlated with physical performance in healthy older individuals and lower leg muscle-specific IMCL differs based on gender. Additionally, the results suggest that greater EMCL content in the soleus and calf subcutaneous fat may positively affect physical performance in women but not men.

Sex-different changes of body composition in aging: a systemic review

Sunyoung Kim et al.

Summary: Aging leads to changes in body composition, specifically differences in fat and muscle distribution and amount, which have varying effects on health outcomes in older men and women. Skeletal muscle mass (sMM) has a positive impact on physical performance, bone density, metabolism, cognitive function, and mortality in both sexes, although the effects are generally smaller in women. Fat mass (FM) is detrimental for physical performance in women, while it can have positive or detrimental effects on glucose level, cognitive function, and lung function depending on the location. The influence of FM in men is usually less significant compared to women in terms of function, bone density, glucose level, and cognitive function.

Relationships between cystatin C- and creatinine-based eGFR in Japanese rural community- dwelling older adults with sarcopenia

Hiroshi Kusunoki et al.

Summary: The study found that eGFRcys and the eGFRcys/eGFRcre ratio were significantly correlated with grip power, gait speed, and skeletal muscle mass index. Additionally, low eGFRcys and low eGFRcys/eGFRcre ratio were associated with a higher risk of sarcopenia in community-dwelling older adults. The eGFRcys/eGFRcre ratio may be a practical screening marker for sarcopenia.

Summary: Population aging has led to an increase in age-related conditions, including sarcopenia and frailty, which are common causes of functional decline in older individuals. This review found that there is a higher prevalence of frailty and sarcopenia in dementia patients, but there is still limited research on how they interrelate with dementia.
