Advances in Epilepsy: Mechanisms, Clinical Trials, and Drug Therapies

Summary: Epilepsy is a neurological disorder characterized by temporary brain dysfunction caused by abnormal neural electrical activity. Drug therapy is the primary method of treatment for epilepsy, but around 30% of patients do not respond to current antiseizure drugs. Long-term use of these drugs can lead to adverse effects and increased tolerability concerns. Research for more effective and safer antiseizure drugs is still an ongoing and urgent task, and this Perspective article focuses on summarizing the current progress of small-molecule drug candidates in epilepsy therapy.

Antinociceptive and Antiallodynic Activity of Some 3-(3-Methylthiophen-2-yl)pyrrolidine-2,5-dione Derivatives in Mouse Models of Tonic and Neuropathic Pain

Summary: Antiseizure drugs (ASDs) are commonly used for nonepileptic conditions, including pain. This study assessed the analgesic effect of four pyrrolidine-2,5-dione derivatives, which have anti-seizure and preliminary antinociceptive activity, in various pain models. The results showed that three compounds demonstrated significant antinociceptive effects in the formalin-induced model of tonic pain. These substances also exhibited antiallodynic properties in a model of peripheral neuropathy. The most active compound did not induce sedative effects at the active dose.

Design, Synthesis and Anticonvulsant Activity of New Phenoxyalkyl, Phenoxyethoxyethyl and Phenoxyacetyl Derivatives of Aminoalkanols

Summary: Forty new aminoalkanol derivatives with potential anticonvulsant activity were designed and synthesized. In vivo studies showed that nineteen compounds exhibited anticonvulsant activity, with compounds 30, 31, and 33 showing relatively low ED50 values and good protective indices. Compounds 30 and 31 showed promising results in mechanism of action investigations and safety assessments.

Summary: In this study, an optimized multistep asymmetric synthesis was conducted to prepare R-enantiomers of a series of racemic hybrid anticonvulsants, with one compound (R)-16 identified as a lead showing broad-spectrum protective activity and low potential for inducing motor impairment. Additionally, (R)-16 exhibited satisfying ADME-Tox properties in vitro, supporting its further preclinical development as a potentially broad-spectrum and safe anticonvulsant candidate.
