Summary: This study evaluated the clinical outcomes of patients undergoing one-step conservative surgery for placenta accreta spectrum in 4 reference hospitals and provided detailed steps for successful application of this type of surgery. Results showed that one-step conservative surgery was possible in 85.3% of cases. Compared to the group managed with cesarean delivery and hysterectomy, the one-step surgery group had lower frequency of transfusions and vascular interventions, and shorter operation time.

Uterine conservative-resective surgery for selected placenta accreta spectrum cases: Surgical-vascular control methods

Summary: The optimal management and vascular control methods in placenta accreta spectrum (PAS) are still controversial. This study compared the maternal outcomes between different vascular control methods in surgical management of PAS. The results showed that uterine conservative-resective surgery had lower blood loss compared with cesarean hysterectomy with internal iliac artery ligation (IIAL). Within the uterine conservative-resective surgery group, identification-ligation of the upper vesical, upper vaginal, and uterine arteries resulted in significantly lower blood loss compared with IIAL. Cesarean hysterectomy is not the default treatment for all PAS cases, but rather uterine conservative-resective surgery with appropriate vascular control may be suitable.

The rail sign: an ultrasound finding in placenta accreta spectrum indicating deep villous invasion and adverse outcomes

Summary: The study found that a rail sign depicted by color Doppler sonography is associated with deeper villous invasion, additional perioperative approaches, and more adverse clinical outcomes. Patients with a rail sign had a significantly higher risk of abnormal placental invasion, such as placenta increta or placenta percreta, and were more likely to require preoperative vascular control, uterine artery embolization, blood transfusions, ICU admission, hysterectomy, and bladder invasion. Their blood loss and hospital stay were also greater than those without a rail sign.
