Immune checkpoint inhibitors and their interaction with proton pump inhibitors-related interstitial nephritis

Summary: Immune checkpoint inhibitors (ICIs) have greatly improved cancer therapy outcomes, but they can also cause immune-related adverse events (irAEs). ICI-associated acute interstitial nephritis (ICI-AIN) is a common irAE that leads to acute kidney injury (AKI) in a small percentage of patients. The pathophysiology of ICI-AIN, particularly its interaction with AIN-inducible drugs like proton pump inhibitors (PPIs), is not yet fully understood. This review aims to discuss the incidence, mechanisms, clinical presentation, diagnosis, and management of PPI-related AIN in the context of ICI therapy.

This study compared patients with renal IRAEs (ICI-AKI) to those with other IRAEs and found that ICI-AKI patients were older, had a higher prevalence of chronic kidney disease, and were more likely to be treated with fluindione, NSAIDs, or PPIs.

Immune checkpoint inhibitor-associated acute kidney injury and mortality: An observational study

Summary: This study aimed to investigate the incidence, risk factors, renal outcomes, and mortality of AKI in patients receiving checkpoint inhibitors for cancer treatment. The study found that one third of AKI cases were associated with checkpoint inhibitors, with an overall incidence of 14.2%. Checkpoint inhibitor-associated AKI was mostly low-grade and resulted in persistent renal dysfunction in approximately 40% of patients. Patients with all-cause AKI had a twofold increased mortality risk, but checkpoint inhibitor-associated AKI did not increase mortality.
