Complete posterior cranial vault distraction osteogenesis to correct Chiari malformation type I associated with craniosynostosis

Hojin Park et al.

Summary: Complete PVDO is an effective method to expand the posterior cranial fossa and alleviate cerebellar tonsillar herniation in patients with craniosynostosis and concurrent CM-I. This study found that complete PVDO significantly increased intracranial volume and posterior skull base area, and reduced cerebellar tonsillar descent in patients.

Morbidity Associated With Anterior Versus Posterior Cranial Vault Expansion for Early Treatment of Syndromic Craniosynostosis: A Systematic Review and Meta-Analysis

Ian J. Richardson et al.

Summary: This systematic review and meta-analysis compared the rates of unplanned reoperation and complications between fronto-orbital advancement and posterior cranial vault expansion as early surgeries for syndromic craniosynostosis. The results showed that anterior cranial vault expansion was associated with significantly higher rates of reoperation and complications compared to posterior cranial vault expansion.

Conventional posterior cranial vault expansion: indications and results-review of the literature

Bienvenido Ros et al.

Summary: For patients with bilateral coronal synostosis, traditional posterior cranial vault expansion surgery is considered safe and provides an immediately remodeled and protective appearance of the skull. However, the underlying cranial base malformation remains untreated postoperatively, which may have implications on the growth of the facial skeleton.

Chiari Malformation Type 1 and Syringomyelia in a Patient With Prior Surgically-Treated Sagittal Synostosis Effectively Treated With Posterior Cranial Vault Distraction Osteogenesis

Christina N. Feller et al.

Summary: This report describes the effective use of posterior cranial vault distraction osteogenesis to address both Chiari malformation and secondary syringomyelia in a child with surgically-treated nonsyndromic craniosynostosis. The procedure showed significant improvement in symptoms and pathological conditions, demonstrating its potential as a treatment strategy for these cases.

A Review on Variability in Treatment Protocols With Posterior Cranial Vault Distraction

Paymon Sanati-Mehrizy et al.

Summary: The study reviewed 24 studies involving 286 patients who underwent PVDO, with an average patient age of 25.34 months. The majority of patients had a latency period of 5-7 days and underwent 1 mm of distraction daily. Sixty complications were reported, resulting in an overall complication rate of 28.3%, with increased patient age correlating with higher complication rates.
