Biomarker-Development Proteomics in Kidney Transplantation: An Updated Review

Summary: Kidney transplantation is the best treatment option for end-stage renal disease patients and requires careful monitoring of graft function. Routine clinical monitoring has limitations and can only detect graft damage at a later stage. Noninvasive biomarker molecules are needed for continuous monitoring after kidney transplantation to enable early diagnosis of graft dysfunction and improve clinical outcomes. Proteomic technologies have revolutionized medical research and have been studied as potential molecular markers for predicting graft outcomes in kidney transplantation. This article reviews the latest findings on proteomic studies in renal transplantation to better understand the potential of this new diagnostic approach.

Urinary Biomarkers for Kidney Allograft Injury

Summary: The current standard of serum creatinine and biopsy has limitations in monitoring allograft health. A biomarker that is sensitive and specific to early detection of allograft injuries is needed. Urine, which directly reflects intrarenal processes, is more accurate than peripheral blood analysis. Transcriptomic, metabolomic, genomic, and proteomic approaches can be used to identify urinary biomarkers for noninvasive detection of allograft injuries, and urine cell-free DNA can be used as a sensitive surrogate for renal allograft biopsy for rejection diagnosis.

Two-Week Protocol Biopsy in Renal Allograft: Feasibility, Safety, and Outcomes

Summary: This study evaluated the technical feasibility, procedure safety, and clinical outcomes of two-week protocol biopsy. The results showed that two-week protocol biopsy is technically feasible and safe, and it contributes to early detection of subclinical rejection. Platelet count and blood urea nitrogen should be carefully checked prior to the biopsy to predict major complications.

Therefore, the development of urinary biomarkers is necessary for the detection of bladder cancer.
