Biomechanical and Mechanobiological Drivers of the Transition From PostCapillary Pulmonary Hypertension to Combined Pre-/PostCapillary Pulmonary Hypertension

Betty J. J. Allen et al.

Summary: Combined pre-/postcapillary pulmonary hypertension (Cpc-PH), a complication of left heart failure, leads to higher mortality rates compared to isolated postcapillary pulmonary hypertension alone. However, little is known about the mechanisms responsible for the progression from isolated postcapillary pulmonary hypertension (Ipc-PH) to Cpc-PH. This review focuses on the biomechanical and mechanobiological impact of left heart failure on pulmonary circulation, including the altered biological signaling and detrimental remodeling that drive the progression to Cpc-PH. Understanding these mechanisms may help identify potential pharmacologic approaches to prevent right heart failure and subsequent mortality.

Pulmonary artery blood flow dynamics in chronic thromboembolic pulmonary hypertension

Hideo Tsubata et al.

Summary: Chronic thromboembolic pulmonary hypertension is caused by incomplete resolution and organization of thrombi. Pulmonary artery blood flow dynamics in patients with this condition have received limited attention, and the effects of treatment interventions on these dynamics are not fully understood. A study used computational fluid dynamics to evaluate the blood flow dynamics in patients with chronic thromboembolic pulmonary hypertension before and after pulmonary endarterectomy and balloon pulmonary angioplasty. Results showed that patients with chronic thromboembolic pulmonary hypertension had decreased flow velocity and wall shear stress, and increased oscillatory shear index and blood stagnation volume, compared to controls. Both treatment interventions improved blood flow dynamics, with the endarterectomy redistributing pulmonary blood flow and the angioplasty decreasing blood flow stagnation and increasing wall shear stress.

Numerical predictions of shear stress and cyclic stretch in pulmonary hypertension due to left heart failure

Michelle A. Bartolo et al.

Summary: Isolated post-capillary pulmonary hypertension (Ipc-PH) is caused by left heart failure, while combined pre-/post-capillary PH (Cpc-PH) is associated with increased mortality. Mechanical forces and biological signaling likely contribute to the transition from Ipc-PH to Cpc-PH. This study used a computational fluid dynamics model to examine the dynamics of mechanical forces in the pulmonary circulation.

Wall shear stress on vascular smooth muscle cells exerts angiogenic effects on extracranial arteriovenous malformations

Jeong Yeop Ryu et al.

Summary: This study found that mechanical shear stress can make vascular smooth muscle cells (VSMCs) in arteriovenous malformations (AVMs) more sensitive, leading to increased angiogenesis. Under shear stress conditions, the expression levels of AGP2, AQP1, and TGFBR1 in AVM tissues were higher than in normal tissues.

Right ventricle remodeling in chronic thromboembolic pulmonary hypertension

Jixiang Liu et al.

Summary: Chronic thromboembolic pulmonary hypertension (CTEPH) is a curable pulmonary vascular disease that is often overlooked. It causes adaptive and maladaptive remodeling of the right ventricle, leading to right heart failure and ultimately death. However, our understanding of the right ventricular remodeling process in CTEPH is currently limited.

Summary: Pulmonary hypertension is a progressive disease that requires multiple tests for diagnosis, with right heart catheterization being the gold standard. Hemodynamic indices show significant changes as the disease progresses. The study aims to identify computational biomarkers as non-invasive alternatives for diagnosing PH.

Summary: The study found that patients with pulmonary arterial hypertension (PAH) have stiffer and larger pulmonary artery walls, reduced relative area change, higher total resistance, and lower total compliance. Additionally, PAH patients have lower wall shear stress values at the main pulmonary arteries.

Accelerated Estimation of Pulmonary Artery Stenosis Pressure Gradients with Distributed Lumped Parameter Modeling vs. 3D CFD with Instantaneous Adaptive Mesh Refinement: Experimental Validation in Swine

Ryan Pewowaruk et al.

Summary: This study compared accelerated predictions of PAS pressure gradients from 3D CFD with AMR and a recently developed 0D lumped parameter CFD model and experimentally validated the predictions using a swine PAS model. The results showed that both 0D and 3D simulations improved efficiency of hemodynamic modeling, but the limited medical imaging resolution caused uncertainty in pressure gradient estimations.

Chronic Thromboembolic Pulmonary Hypertension - What Have We Learned From Large Animal Models

Kelly Stam et al.

Summary: CTEPH develops in some patients after acute pulmonary embolism, leading to increased pulmonary vascular resistance and right ventricle dilation. The mechanisms of pulmonary microvascular remodeling, processes underlying RV structural and functional adaptability, and determinants of arrhythmia susceptibility in CTEPH are not fully understood. Large animal models have been developed to mimic key clinical features of CTEPH and subsequent RV remodeling, aiding in understanding these processes.

A multiscale model of vascular function in chronic thromboembolic pulmonary hypertension

Summary: This study presents a novel computational framework for predicting pulmonary hemodynamics in chronic thromboembolic pulmonary hypertension (CTEPH) and identifying optimal target lesions for balloon pulmonary angioplasty (BPA) therapy. The research uses a multiscale approach, incorporating simulated pulmonary artery pressure, wave intensity analysis, and a new quantitative metric of flow heterogeneity to personalize treatment for CTEPH patients who are not suitable for surgery.

A computational model of contributors to pulmonary hypertensive disease: impacts of whole lung and focal disease distributions

Summary: Pulmonary hypertension, with multiple etiologies, presents challenges in diagnosis, prognosis, and treatment. Invasive hemodynamic measurements provide diagnosis but dynamic assessment is needed to differentiate disease phenotypes. Models can help understand differences in pulmonary vascular response, but non-linear relationships between data and structural changes may complicate analysis for distinguishing between cohorts.
