Development and Validation of a Genomic Profile for the Omission of Local Adjuvant Radiation in Breast Cancer

Martin Sjoestroem et al.

Summary: This study aimed to identify women with low risk of recurrence who may safely be spared adjuvant radiotherapy (RT) using a molecular-based approach. By analyzing two randomized trials, a genomic signature named POLAR was trained to predict locoregional recurrence (LRR) and guide the decision of omitting RT in patients with low risk of LRR.

Patient-reported outcomes for the Intergroup Sentinel Mamma study (INSEMA): A randomised trial with persistent impact of axillary surgery on arm and breast symptoms in patients with early breast cancer

Toralf Reimer et al.

Summary: The INSEMA trial evaluated the non-inferiority of avoiding sentinel lymph node biopsy (SLNB) or axillary lymph node dissection (ALND) in clinically node-negative breast cancer patients. Patient-reported outcomes (PROs) were assessed using the EORTC QLQ-C30 and BR23 questionnaires. The study found that patients with no SLNB had better outcomes in terms of breast and arm symptoms, while there were no relevant differences in other scales.

Evolution and refinement of magnetically guided sentinel lymph node detection in breast cancer: meta-analysis

Eirini Pantiora et al.

Summary: Superparamagnetic iron oxide nanoparticles (SPIO) have been used as a tracer for sentinel lymph node (SLN) localization in breast cancer, demonstrating comparable performance to the combination of radioisotope (RI) and blue dye (BD). The detection rate for SPIO is slightly higher than RI +/- BD, and preoperative injection over 24 hours increases the detection rate.

Diagnostic performance of image-guided vacuum-assisted breast biopsy after neoadjuvant therapy for breast cancer: prospective pilot study

Elisabetta M. C. Rossi et al.

Summary: This study assessed the accuracy of preoperative VABB specimens compared to surgical specimens in patients with triple-negative or HER2-positive breast cancer after neoadjuvant therapy. The performance of contrast-enhanced MRI and PET-CT for response prediction was also evaluated.

No axillary surgical treatment for lymph node-negative patients after ultra-sonography [NAUTILUS]: protocol of a prospective randomized clinical trial

Ji Gwang Jung et al.

Summary: This article introduces a trial on the application of sentinel lymph node biopsy (SLNB) in early breast cancer patients. The trial aims to evaluate whether SLNB can be omitted in patients undergoing breast-conserving surgery and receiving whole-breast radiation if the axillary lymph node is not suspicious during preoperative imaging. The trial will assess patients' survival rates, distant metastasis rate, axillary recurrence rate, and quality of life.

Intelligent Vacuum-Assisted Biopsy to Identify Breast Cancer Patients With Pathologic Complete Response (ypT0 and ypN0) After Neoadjuvant Systemic Treatment for Omission of Breast and Axillary Surgery

Andre Pfob et al.

Summary: Neoadjuvant systemic treatment is effective for breast cancer patients, but current nonsurgical methods cannot accurately identify patients without residual cancer. We developed an intelligent vacuum-assisted biopsy approach using machine learning algorithm, which can reliably exclude residual cancer.

Summary: This study aims to evaluate whether radiotherapy alone can replace breast surgery in early-stage triple-negative breast cancer or HER2-positive breast cancer patients who have achieved pathological complete response through image-guided vacuum-assisted core biopsy.

Factors Associated with Axillary Lymph Node Status in Clinically Node-Negative Breast Cancer Patients Undergoing Neoadjuvant Chemotherapy

Chi-Chang Yu et al.

Summary: This study aimed to investigate the factors associated with axillary lymph node (ALN) status in breast cancer patients initially diagnosed with clinically node-negative (cN0) tumors who underwent neoadjuvant chemotherapy (NAC). The results showed that achieving a pathologic complete response in the breast was associated with negative ALN status, while the presence of lymphovascular invasion was associated with positive ALN status.

Ultrasound-assisted carbon nanoparticle suspension mapping versus dual tracer-guided sentinel lymph node biopsy in patients with early breast cancer (ultraCars): phase III randomized clinical trial

Toward omitting sentinel lymph node biopsy after neoadjuvant chemotherapy in patients with clinically node-negative breast cancer

M. E. M. van der Noordaa et al.

Summary: In patients with clinically node-negative breast cancer, the rate of achieving ypN0 after neoadjuvant chemotherapy is low. The study identifies characteristics known before surgery that are associated with achieving ypN0, which can help select patients for omitting sentinel lymph node biopsy.

Minimally Invasive Complete Response Assessment of the Breast After Neoadjuvant Systemic Therapy for Early Breast Cancer (MICRA trial): Interim Analysis of a Multicenter Observational Cohort Study

Ariane A. van Loevezijn et al.

Summary: The study found that ultrasound-guided core biopsies are not accurate enough to identify breast pCR in patients with good response on MRI after neoadjuvant systemic therapy. Therefore, breast surgery cannot be safely omitted based solely on the results of core biopsies in these patients.

Identification of breast cancer patients with pathologic complete response in the breast after neoadjuvant systemic treatment by an intelligent vacuum-assisted biopsy

Summary: The study developed and tested multivariate algorithms to accurately identify breast cancer patients without residual cancer after neoadjuvant treatment. The neural network showed the best performance with a specificity ranging from 65.2% to 67.2% in both test and external validation datasets.
