Summary: This study evaluated the feasibility of routine nutrition risk screening in community-dwelling older adults using a partnership between health care and community-based organizations. The results showed that the partnership was able to provide screening and appropriate referrals for at-risk individuals. However, sustainability requires strengthening of community resources, referral processes, and telephone assessments.

Consensus-Based Nutrition Care Pathways for Hospital-to-Community Transitions and Older Adults in Primary and Community Care

Summary: This study developed evidence- and consensus-based nutrition care pathways for adult patients at risk for malnutrition transitioning from hospital to community, with input from diverse stakeholders. Survey results showed high ratings of >= 80% for relevance and importance of most nutrition care practices. These pathways offer flexible guidance for different primary care and community models.

Sample sizes for saturation in qualitative research: A systematic review of empirical tests
