Review on host-pathogen interaction in dermatophyte infections

Chhavi Gupta et al.

Summary: Dermatophytosis is a common fungal infection of the skin caused by dermatophytes. The proteolytic enzymes released by the fungi contribute to the persistence of infection. Abnormal host immune response leads to inflammation in chronic dermatophytosis. Increasing antifungal resistance and recalcitrant infections pose challenges in clinical management.

Single-Point Nail Sampling to Diagnose Onychomycosis Caused by Non-Dermatophyte Molds: Utility of Polymerase Chain Reaction (PCR) and Histopathology

Aditya K. Gupta et al.

Summary: The most commonly used methods for diagnosing non-dermatophyte mold (NDM) onychomycosis are culture, polymerase chain reaction (PCR), and histopathology. A study compared the results of all three diagnostic tests on toenail samples from 512 suspected cases of onychomycosis. The study found a significant association between PCR and histopathology results, as well as between fungal culture and histopathology results. The overall rate of dermatophyte detection was higher using PCR compared to culture.

Deep learning-based diagnosis models for onychomycosis in dermoscopy

Xianzhong Zhu et al.

Summary: This study explored the establishment of deep learning-based diagnostic models for onychomycosis in dermoscopy and found that the deep learning-based diagnosis models showed superior diagnostic accuracy for onychomycosis compared to dermatologists. The distinctive dermoscopic patterns of onychomycosis were identified, and the deep learning models were more accurate in diagnosing the condition.

Summary: PCR method is better for detecting fungal infection, especially non-dermatophyte molds, compared to the traditional KOH/culture method. It can detect more non-dermatophyte molds before treatment and after treatment. PCR method is more reliable for evaluating the effectiveness of topical treatment for onychomycosis.

Summary: The prevalence rate of onychomycosis in this study was found to be high, with a higher isolation rate for non-dermatophyte molds compared to dermatophytes and yeasts. The dominant etiological agents were Trichophyton interdigitale, Aspergillus spp, and Candida albicans. Females and patients in the middle age group were more affected by onychomycosis.

Summary: In this study, a machine learning algorithm was used to detect fungal elements on digitized histologic sections of human nail specimens, and its performance was compared with that of dermatopathologists. The results demonstrated that the algorithm achieved a comparable sensitivity to the human pathologists. This finding suggests that machine learning algorithms can serve as supportive diagnostic tools to improve the screening efficiency of fungal infections.

Summary: Onychomycosis is a common nail disease that poses a major public health issue worldwide. Dermatophytes are the main cause of onychomycosis, but there is an increasing trend in yeast and non-dermatophyte mold infections. Lack of knowledge about onychomycosis leads to underdiagnosis and the spread of fungi among individuals and susceptible populations.
