Characterization of multi-DAA resistance using a novel hepatitis C virus genotype 3a infectious culture system

Carlota Fernandez-Antunez et al.

Summary: Understanding the viral determinants that influence antiviral resistance is crucial for preventing the emergence of resistance to direct-acting antiviral treatments. It has been found that NS5A-inhibitor resistance plays a significant role in treatment failure of HCV genotype 3.

Hepatitis C virus RNA is 5'-capped with flavin adenine dinucleotide

Anna V. Sherwood et al.

Summary: A new study reveals that hepatitis C virus (HCV) RNA possesses a non-canonical 5' cap, consisting of the cellular metabolite flavin adenine dinucleotide (FAD). This FAD capping strategy allows HCV RNA to evade immune recognition while maintaining its stability, potentially influencing the efficacy of antiviral treatments and the persistence of infection.

HCV genome-wide analysis for development of efficient culture systems and unravelling of antiviral resistance in genotype 4

Long V. Pham et al.

Summary: HCV genotype 4 infections are a major cause of liver diseases in the Middle East/Africa, with certain subtypes associated with increased risk of direct-acting antiviral treatment failures. Developing infectious genotype 4 cell culture systems can help understand the evolutionary genetic landscapes of antiviral resistance and preserve the future efficacy of DAA-based therapy.

Novel HCV Genotype 4d Infectious Systems and Assessment of Direct-Acting Antivirals and Antibody Neutralization

Long Pham et al.

Summary: Hepatitis C virus (HCV) genotype 4 is highly prevalent in the Middle East and parts of Africa. Subtype 4d has recently spread among high-risk groups in Europe. However, 4d infectious culture systems are not available, hindering drug studies and neutralizing antibody research relevant for HCV vaccine development. This study developed the first genotype 4d infectious culture system, enabling efficacy testing of direct-acting antivirals (DAAs) and assessment of antibody neutralization, which is critical for optimizing DAA treatments and designing vaccines to combat the HCV epidemic.

Summary: This study reports an efficient infectious cell culture model for hepatitis C virus (HCV), which can be used for studying infection mechanisms and developing vaccines. The model, generated from a consensus gt1b genome, underwent long-term passaging to adapt and produce high levels of transmissible infectivity. The study's findings are important for understanding the transmission mechanisms of HCV and for research and prevention of HCV.

Global evolutionary analysis of chronic hepatitis C patients revealed significant effect of baseline viral resistance, including novel non-target sites, for DAA-based treatment and retreatment outcome

Ulrik Fahnoe et al.

Summary: The study found that resistance-associated substitutions (RASs) were associated with treatment failure in HCV patients receiving direct-acting antivirals. Patients with treatment failure had a higher proportion of HCV NS5A-RASs compared to sustained virologic response (SVR) patients. Treatment failure was also related to slower declines in viral titres and decreased viral population diversity possibly caused by selection for RASs.

The Role of RASs/RVs in the Current Management of HCV

Konstantinos Malandris et al.

Summary: The approval of combination therapies with direct-acting antiviral (DAA) regimens has led to significant progress in the field of hepatitis C virus (HCV) treatment. However, resistance to DAAs is a major issue, with resistance-associated substitutions (RASs) having an impact on sustained virological response (SVR) rates. Different RASs in NS3, NS5A and NS5B are linked with reduced susceptibility to DAAs, and DAA-resistant HCV variant haplotypes (RVs) are dominant in cases of virological failure.

David A. Smith et al.

Summary: Researchers identified three common polymorphisms in non-targeted HCV NS2 and NS3 proteins associated with reduced treatment response to sofosbuvir in hepatitis C virus infection. These polymorphisms do not affect sofosbuvir potency but may impact treatment outcomes through an indirect mechanism.

Summary: Hepatitis C virus is highly diverse, with subtypes that are either prevalent globally or have limited distribution. Genotype 3-hepatitis C virus' last line of defense

Dorota Zarebska-Michaluk

Summary: Chronic infection with hepatitis C virus (HCV), particularly genotype 3 (GT3), can result in higher rates of steatosis, faster progression of liver fibrosis, and lower efficacy of antiviral treatment compared to other HCV genotypes. The treatment options have evolved from interferon-based regimens to highly potent interferon-free pangenotypic direct-acting antivirals (DAA), but there is still room for improvement, especially in patients with liver cirrhosis and those who fail to respond to DAA therapies.

Summary: The study aimed to compare 4 weeks of glecaprevir/pibrentasvir (GLE/PIB) treatment with and without ribavirin for patients with chronic hepatitis C and favourable baseline characteristics. Results showed that baseline RAS were more frequent in patients with virological failure. All patients who failed treatment were cured after retreatment with a sofosbuvir-based regimen.
