Summary: This document provides a comprehensive review of the current status of ultrasound-guided ablation procedures for benign and malignant thyroid conditions, along with 18 best practice recommendations in various topic areas. As ultrasound-guided ablation procedures become more common in thyroid disease treatment, evidence-based and thoughtful implementation of best practices is essential.

Cooling Effects Occur in Hepatic Microwave Ablation At Low Vascular Flow Rates and in Close Proximity to Liver Vessels - Ex Vivo

Franz G. M. Poch et al.

Summary: This study systematically assessed the vascular cooling effects in hepatic microwave ablation ex vivo. The results showed that the vascular cooling effect was mainly affected by the distance between the antenna and the vessel, while vessel diameter and flow rate played a minor role in vascular cooling effects.

The Ablation of Thyroid Nodule's Afferent Arteries Before Radiofrequency Ablation: Preliminary Data

Chiara Offi et al.

Summary: Induced radiofrequency thermal ablation is a safe and effective alternative treatment for symptomatic benign thyroid nodules, metastatic and recurrent thyroid tumors, and papillary thyroid microcarcinomas. The artery-first feeding radiofrequency ablation technique may lead to improved results in terms of nodule reduction. Further research and longer follow-up are needed to confirm these findings.

Long-Term Outcomes of Thermal Ablation for Benign Thyroid Nodules: The Issue of Regrowth

Suk Sim Jung et al.

Summary: Thermal ablation for benign thyroid nodules is considered an effective and safe alternative to surgery, but long-term studies have shown issues with nodule regrowth and symptom recurrence, highlighting the need for better control. A more comprehensive and long-lasting treatment effect may be necessary for better outcomes.

Radiofrequency Thermoablation On Ex Vivo Animal Tissues: Changes on Isolated Swine Thyroids

Paola Pregel et al.

Summary: The study compared the size of ablation zones induced by RFA systems using internally cooled perfused vs. non-perfused electrodes in swine thyroids. Results showed that perfused electrodes, especially with 7.0% saline solution, produced larger ablation zones compared to non-perfused electrodes. This increase in electrical conductivity of the tissue resulted in more effective ablation of thyroid nodules.

Summary: With the increasing detection of papillary thyroid microcarcinomas (PTMCs) and relapsing cervical differentiated thyroid carcinoma (DTC) metastases, active surveillance (AS) and minimally invasive treatments (MITs) are being proposed as alternatives to surgery and radioiodine. In certain cases, a multidisciplinary team assessment is needed to appropriately select patients for MIT.
