Summary: Occult breast cancer with axillary metastasis is rare and challenging to diagnose and treat. The evidence for clinical management is limited and the approach to local treatment varies greatly. Historically, modified radical mastectomy with radiotherapy was used, but breast preservation with adjuvant whole breast radiotherapy achieves similar outcomes. Axillary lymph node dissection is the standard surgical approach, but de-escalating axillary surgery may be appropriate for select patients who respond well to neoadjuvant chemotherapy. The integration and customization of axillary surgery and regional nodal radiotherapy remains controversial and under active investigation.
