Pregnancy outcome after cesarean section following a failed vacuum attempt

Summary: This study compared the pregnancy outcomes of women who underwent cesarean section in the second stage of labor, with or without a vacuum extraction attempt. The results showed that failed vacuum attempts were associated with increased neonatal morbidity, but not increased maternal morbidity.

Impacted fetal head during second stage Caesarean birth: A prospective observational study

Summary: This study investigated the incidence and complication rates of impacted fetal head at full dilatation Caesarean birth in the UK, as well as the techniques used. It was found that 16% of cases involved the use of a dis-impaction technique or reported difficulty in delivering the head. The most commonly used techniques were manual elevation of the head through the vagina and the use of a fetal pillow. Unfortunately, 2% of babies died or sustained severe injury.

Severe maternal and neonatal morbidity after attempted operative vaginal delivery

Summary: Successful operative vaginal delivery was associated with a 45% reduction in severe maternal morbidity without differences in severe unexpected neonatal morbidity compared with cesarean delivery after prolonged second stage of labor. Failed operative vaginal delivery was associated with increased severe maternal morbidity and severe unexpected neonatal morbidity, with combined operative vaginal deliveries contributing significantly to these adverse outcomes. Efforts to optimize operative vaginal delivery success rates should focus on improved patient selection, enhanced provider skill, and avoiding combined operative vaginal deliveries to reduce maternal and neonatal complications.
