Genotypes and phenotypes heterogeneity in PIK3CA-related overgrowth spectrum and overlapping conditions: 150 novel patients and systematic review of 1007 patients with PIK3CA pathogenetic variants

Summary: In this study, the disease heterogeneity of PIK3CA-related overgrowth spectrum (PROS) was explored, and the factors contributing to the diseases were identified. By analyzing 150 new patients and 1007 previously reported mutated patients, a comprehensive picture of the diseases was obtained. The study found that strongly activating PIK3CA variants were mainly present in affected tissues, while weakly activating variants were mainly present in blood. Additionally, strongly activating variants were more common in cases without central nervous system involvement, while weakly activating variants were more associated with central nervous system involvement.

Undergrowth Of First Toe In PiK3CA-Related Overgrowth Spectrum (PROS)

Summary: PROS is a heterogeneous group of disorders characterized by segmental overgrowth secondary to somatic mosaic activating variants in PIK3CA. In this study, we observed a group of PROS patients with undergrowth associated with their focal overgrowth. Retrospective review showed that all patients presented lower limb overgrowth and undergrowth of the ipsilateral first toe, and confirmed PIK3CA pathogenic variants were found in all patients. Despite receiving alpelisib treatment, there was partial response in lipomatosis and vascular anomalies but no response in overgrowth and undergrowth.

Segmental undergrowth is associated with pathogenic variants in vascular malformation genes: A retrospective case-series study

Summary: Segmental undergrowth has been poorly characterized in patients with congenital vascular anomalies, and this study presents clinical and molecular characteristics of 37 patients with vascular malformations and undergrowth. Deep sequencing identified different groups associated with specific vascular malformations and detected likely pathogenic variants in certain genes. The study concludes that segmental undergrowth should be considered a characteristic associated with vascular malformations, and the term PROS does not cover the entire spectrum of overgrowth and undergrowth patterns in patients with PIK3CA variants.

Somatic mutational landscape of extracranial arteriovenous malformations and phenotypic correlations

Summary: This study characterizes the somatic genetic landscape of extracranial arteriovenous malformations and identifies a correlation between genotype, clinical severity, and angiographic characteristics. Pathogenic somatic variants, predominantly in MAP2K1 and KRAS genes, were found with high prevalence. These findings could lead to new therapeutic options for extracranial arteriovenous malformations.

A pilot study of next generation sequencing-liquid biopsy on cell-free DNA as a novel non-invasive diagnostic tool for Klippel-Trenaunay syndrome

Summary: Our study conducted a comprehensive analysis of seven patients with Klippel-Trenaunay syndrome and found pathogenic mutations in the PIK3CA gene in all patients using cell-free DNA analysis. Tissue analysis confirmed the presence of the mutation at the malformation site in one amputated patient at a high molecular frequency, indicating its causative role. This study demonstrates that cell-free DNA-next generation sequencing-liquid biopsy can be an effective tool for diagnosis and personalized treatment of Klippel-Trenaunay syndrome.

The differential diagnosis of VM is challenging, except for diseases with pathognomonic phenotypes, but increasing sequencing techniques will facilitate this process. Targeted pharmacotherapy shows promise for the treatment of VM patients.

Clinical experience with the AKT1 inhibitor miransertib in two children with PIK3CA-related overgrowth syndrome

Summary: This case series presents the use of miransertib in two pediatric patients with PROS, showing objective clinical response in one patient and improvement in key qualitative outcomes in another. Treatment was well tolerated with no significant toxicities reported, highlighting the potential therapeutic utility of miransertib in selected pediatric patients with severe PROS and demonstrating the possibility of repurposing targeted therapies for rare diseases. An open label, Phase 1/2 study of miransertib in children with PROS and PS is currently underway to more accurately assess its efficacy in treating PROS disorder.

Alpelisib administration reduced lymphatic malformations in a mouse model and in patients

Genetic landscape of common venous malformations in the head and neck

Summary: This study reviewed patients from eight sequencing studies related to common VMs and found that the total prevalence rates of TEK and PIK3CA mutations were 41.3% and 26.7%, respectively. The most frequent locations for VMs in the head and neck were the maxillofacial region and the oral and oropharyngeal region. Targeted sequencing identified TEK variants in 21.4% of cases, suggesting a unique genetic landscape for common VMs in the head and neck.
