Beyond the Gut: A Systematic Review of Oral Manifestations in Celiac Disease

Alberta Lucchese et al.

Summary: This systematic review examines oral manifestations in patients with celiac disease. After analyzing 33 selected articles out of 209 identified in the initial search, it is found that oral manifestations in celiac patients include recurrent aphthous stomatitis (34.6%), atrophic glossitis and geographic tongue (15.26%), enamel defects (42.47%), delayed dental eruption (47.34%), xerostomia (38.05%), glossodynia (14.38%), and other manifestations. These findings highlight the importance of oral manifestations in diagnosing celiac disease.

Dental Manifestations and Celiac Disease-An Overview

Herbert Wieser et al.

Summary: This review summarizes recent investigations on dental manifestations in celiac disease, including delayed dental eruption and maturity, dental enamel defects, molar incisor hypomineralization, dental caries, dental plaque, and periodontitis. Studies have shown a higher frequency of delayed dental eruption and maturation in children, and dental enamel defects in individuals with celiac disease compared to healthy individuals. Malabsorption of micronutrients and impaired immunity are considered the main causes. Early diagnosis and a gluten-free diet can prevent the development of these conditions. Dentists can play a significant role in identifying unrecognized celiac disease and preventing long-term complications. Research on dental caries, plaque, and periodontitis in celiac disease is lacking and inconsistent, requiring further examination.

Salivary gland involvement and oral health in patients with coeliac disease

Jason Liu et al.

Summary: This study found that patients with coeliac disease are more likely to experience dry mouth, mucosal lesions, dry/cracked lips, and focal lymphocytic sialadenitis. However, there is no significant difference in dental health and levels of cariogenic bacteria and Candida compared to healthy controls.

Serum and salivary tissue transglutaminase IGA (tTG-IGA) level in celiac patients

Mehran Ajdani et al.

Summary: This study suggests a non-invasive method for diagnosing CD by comparing salivary and serum levels of tTG-IgA. It also highlights the importance of managing dry mouth in CD patients to prevent complications. Overall, CD can affect salivary enzymes and elements, increase inflammatory cytokines, and affect tTG levels in saliva and serum.

Immune-Mediated Association Between Celiac Disease and Sensorineural Hearing Loss: A Systematic Narrative Review

Pietro De Luca et al.

Summary: This review examines the correlation between auto-immune sensorineural hearing loss and celiac disease and provides insights for future research directions. Although some studies suggest that celiac disease may cause subclinical hearing loss, the pathogenic mechanisms and specific link between the two conditions remain unclear. Further rigorous studies are needed to better understand the relationship between hearing loss and celiac disease.

Summary: This scholarly review examines the extraintestinal ENT manifestations of celiac disease, highlighting the importance of awareness and correct diagnosis by otolaryngologists. The review identifies various manifestations, such as hearing loss and nasal septal perforation, and suggests that a gluten-free diet may help alleviate symptoms in most cases.

Dental enamel defects and oral cavity manifestations in Asian patients with celiac disease

Anam Ahmed et al.

Summary: The study found a high prevalence of dental enamel defects and recurrent aphthous ulcers in Asian patients with CeD, suggesting oral issues may be an indicator of CeD. The dry mouth sensation was also significantly higher in CeD patients. Therefore, dental assessment and attention to oral problems in CeD patients are important.

Oral manifestations of celiac disease in French children

L. Villemur Moreau et al.

Summary: Celiac disease is an immune-mediated disorder caused by gluten ingestion, with gastrointestinal and oral manifestations. Children with CD have significantly higher rates of dental enamel defects and recurrent aphthous stomatitis compared to the control group. Early detection of these oral manifestations can help improve detection of CD.

Celiac Disease Causes Epithelial Disruption and Regulatory T Cell Recruitment in the Oral Mucosa

Javier Sanchez-Solares et al.

Summary: The study found that CD patients have significantly decreased expression of epithelial junction proteins in their oral mucosa, suggesting compromised barrier integrity. The increase in FoxP3+ cells in CD patients indicates recruitment of Tregs to the damaged mucosa. Amphiregulin mRNA levels in PBMCs and epithelial damage in the oral mucosa correlated with Treg infiltration, suggesting a potential repair phenotype.

Summary: This study compared the oral manifestations of children with celiac disease (CD) against healthy controls in Saudi Arabia. Children with CD had more recurrent aphthous stomatitis (RAS) and dental enamel defects (DEDs) compared to healthy controls. Pediatric dentists should consider the possibility of CD in child patients presenting with RAS or DEDs.
