Atypical follicular hyperplasia with light chain-restricted germinal centers after COVID-19 booster: a diagnostic pitfall

Summary: COVID-19 vaccine can cause lymphadenopathy which may resemble metastasis or cancer recurrence. This case report describes the histopathologic features of persistent lymphadenopathy in a breast cancer patient following a COVID-19 vaccine booster. Although the histological features mimic follicular lymphoma, the findings were ultimately interpreted as a reactive lesion associated with the timing of COVID-19 vaccine administration, rather than true lymphoma.

Immune Response Related to Lymphadenopathy Post COVID-19 Vaccination

Summary: Mass vaccination against COVID-19 has led to frequent reports of vaccine-associated lymphadenopathy, which is influenced by factors such as age, gender, and lymph node reaction. A systematic review of sixty-two articles revealed that days post-vaccination and B cell germinal center response are negatively associated with the incidence of lymphadenopathy. The study suggests that a strong vaccine immune response, possibly mediated by the B cell germinal center response, contributes to the development of lymphadenopathy. Therefore, it is important to differentiate between reactive lymph nodes and metastatic lymph node enlargement, especially in patients with underlying malignancy, through medical history collection or evaluation.

Extrafollicular proliferation of B-blasts: Morphologic correlate to Spikevax-induced lymphadenopathy

Summary: A 30-year-old male presented with PET-positive left-sided cervical lymphadenopathy, suspected to be metastasis of a known right-sided papillary thyroid cancer. The first dose of Pfizer's Spikevax vaccine administered three weeks ago was not documented or reported to the pathologists. Diagnostic lymphadenectomy revealed extrafollicular proliferation of B-blasts, likely associated with the vaccine administration.

Pathological Findings in COVID-19 and Non-COVID-19 Vaccine-Associated Lymphadenopathy: A Systematic Review

Summary: This study described the pathological findings of COVID-19 vaccine-associated lymphadenopathy and non-COVID-19 vaccine-associated lymphadenopathy. Most cases of COVID-19 vaccine-associated lymphadenopathy were diagnosed as reactive or benign, but other diagnoses such as florid lymphoid hyperplasia, Kikuchi-Fujimoto disease, and granulomatous inflammation were also reported. Clinical and radiological correlation is needed for optimal patient management in most cases.

Multifocal lymphadenopathies with polyclonal reactions primed after EBV infection in a mRNA-1273 vaccine recipient

Summary: We report a case of recurrent tender, multifocal lymphadenopathies in an mRNA-1273 vaccine recipient who recently had an Epstein-Barr virus infection. The clinical presentation mimicked lymphoma. Lymph node biopsy revealed monocytoid B-cell hyperplasia, TH2 (GATA3+) predominance, and hyperplasia of interferon-gamma-producing plasmacytoid dendritic cells. Sustained neutralising antibody production against SARS-CoV-2 wild-type and five variants was observed. The patient exhibited high titres of anti-S antibodies and neutralising antibodies, except for variant B.1.529** (omicron) and B.1.351** (beta) due to spike protein mutations. We hypothesize that EBV, in combination with the mRNA-1273 vaccine, induced a prolonged inflammatory response.
