Retethering risk in pediatric spinal lipoma of the conus medullaris

Toshiaki Hayashi et al.

Summary: In this study, a retrospective analysis of 51 pediatric patients with LCM who underwent untethering surgery and were followed for over 100 months revealed that more complex lesions and a high patient growth rate after surgery seemed to indicate increased risk of ReTCS.

Clinical and urodynamic features of secondary tethered cord syndrome: How can they be found longitudinally?

Su B. Lee et al.

Summary: The study revealed that deteriorating symptoms in the urological system should raise suspicion of secondary tethered cord syndrome (TCS), but reuntethering surgery can effectively address these issues. Additionally, some patients might experience neuro-orthopedic complications despite improvements in urological functions.

Spinal column shortening for tethered cord syndrome: a systematic review and individual patient data meta-analysis

Luke G. McVeigh et al.

Summary: Spinal column shortening (SCS) is an alternative treatment for tethered cord syndrome (TCS) that shows potential in improving symptoms and reducing complications. This systematic review and meta-analysis evaluates the safety and efficacy of SCS for TCS based on available data. The results suggest that SCS can be considered a safe and efficacious treatment option, particularly for complex and recurrent cases.

A systematic review and meta-analysis of minimally invasive surgery in children with occult tethered cord syndrome

Ketao Xu et al.

Summary: This study used systematic review and meta-analysis to evaluate the effect of minimally invasive surgery on children with occult tethered cord syndrome. The results showed that minimally invasive surgery was more effective than conservative treatment, but further evaluation of treatment risks is needed due to the small sample size.

Summary: The purpose of this study was to evaluate the outcomes of surgical transection of filum terminale (FT) in pediatric patients with tethered cord syndrome (TCS). The study found that symptomatic patients experienced neurological improvement or halted deterioration after surgery, while asymptomatic patients did not have any severe complications.

Early de-tethering: analysis of urological and clinical consequences in a series of 40 children

Laura Grazia Valentini et al.

Summary: The study suggests that early de-tethering procedures may prevent or reverse spontaneous neuro-urological deterioration, but in high risk cases (chaotic lipoma and terminal myelocystocele), there is a significant risk of irreversible preoperative urodynamic impairment. The use of intraoperative neurophysiological monitoring (IOM) was found to be more effective in protecting motor functions than neuro-urological functions.

Diagnostic value of Knosp grade and modified Knosp grade for cavernous sinus invasion in pituitary adenomas: a systematic review and meta-analysis

Yi Fang et al.

Summary: The study found that grades 2-4, 3-4, and 3B-4 of the Knosp grade had good discriminative abilities in predicting CSI, and modifying the KG improved its prognostic role in CSI and gross total resection. However, the reliability of the scale's middle grades (grades 2 and 3A) was weak, and therefore these grading systems cannot be used as a standard for invasive and non-invasive pituitary adenomas. It is recommended that future studies report KG as high, medium, and low to optimize the application of the scale.

Prognostication of the neurological outcome of tethered cord based on intraoperative neuromonitoring findings: how close can we get?

Suhas Udayakumaran et al.

Summary: This study evaluated the correlation between intraoperative neuromonitoring data and neurological outcomes in children undergoing surgery for tethered cord syndrome. Most patients with motor, bladder, and bowel disturbances showed improvement at one-year follow-up after surgery. Neuromonitoring complements preoperative clinical details in predicting immediate and long-term outcomes.

Features of the Filum Terminale in Tethered Cord Syndrome with Focus on Pathology

Jungbo Sim et al.

Summary: This study analyzed the characteristics of patients who underwent transection of the filum, finding interesting correlations between pathological findings and clinical features. Patients with glial cells were more likely to have no preoperative syrinx, and symptomatic patients had a larger percentage of peripheral nerve twigs compared to asymptomatic patients. Surgical outcomes for syrinx were favorable, with the exception of one patient whose syrinx increased in size due to upward displacement of the cut filum.
