Increased intra-abdominal haemorrhage after thoracic endovascular aortic replacement in patients with concomitant blunt aortic injuries and abdominal trauma: a National Trauma Data Bank analysis

Yu-Chieh Lu et al.

Summary: This study evaluated the role of TEVAR in patients with concomitant BTAI and BAT, and found that there is a higher risk of delayed abdominal surgeries and blood transfusion after TEVAR. Among patients who underwent TEVAR, those with severe abdominal injury were more likely to have delayed abdominal surgeries for haemostasis.

Summary: This study found that patients with concomitant blunt thoracic aortic injury and abdominal trauma who underwent thoracic endovascular aortic repair (TEVAR) had an increased risk of delayed intra-abdominal hemorrhage.

Timing of repair of blunt thoracic aortic injuries in the thoracic endovascular aortic repair era

Abdul Q. Alarhayem et al.

Summary: The majority of BTAI patients undergo TEVAR repair within 24 hours of injury, with delayed repair patients showing improved survival compared to those repaired within the first 24 hours. Delaying endovascular repair beyond 24 hours after injury should be considered for BTAI patients without signs of imminent rupture for better outcomes.

Summary: Intraoperative heparin use is associated with reduced inpatient mortality for patients undergoing TEVAR for BTAI, and does not increase the risk of hemorrhagic complications.

Efficacy and Safety of Heparinization before Deployment of Endograft for Blunt Traumatic Aortic Injury in Severely Injured Patients

Vladimir Makaloski et al.

Summary: This study reports the early outcomes of patients undergoing thoracic endovascular aortic repair (TEVAR) for blunt traumatic aortic injury (BTAI) and evaluates the use and doses of unfractionated heparin (UFH) during the procedure. The results suggest that systemic heparinization in different doses during TEVAR for BTAI can be safe with no intraoperative bleeding or thromboembolic complications in the early postoperative period.
