Dose-dependent relationship between intra-procedural hypoxaemia or hypocapnia and postoperative delirium in older patients

Elena Ahrens et al.

Summary: This study investigated the association between intra-procedural hypoxaemia or hypocapnia and delirium after surgery. The results showed that both hypoxaemia and hypocapnia were associated with an increased risk of postoperative delirium, and this association was dependent on the duration and magnitude of the events. Therefore, maintaining normal gas exchange is important to prevent postoperative neurological disorders.

Summary: This study suggests that higher mechanical power during ventilation is associated with a greater risk of postoperative respiratory failure requiring reintubation in patients undergoing general anesthesia.

Association between intraoperative tidal volume and postoperative respiratory complications is dependent on respiratory elastance: a retrospective, multicentre cohort study

Aiman Suleiman et al.

Summary: The association of harm with higher tidal volumes during intraoperative mechanical ventilation is modified by respiratory system elastance. These data suggest that respiratory elastance should inform the design of perioperative trials testing intraoperative ventilatory strategies.

Association Between Intraoperative Arterial Hypotension and Postoperative Delirium After Noncardiac Surgery: A Retrospective Multicenter Cohort Study

Luca J. Wachtendorf et al.

Summary: The study found that intraoperative arterial hypotension is associated with an increased risk of postoperative delirium in noncardiac surgical patients, and this association is dependent on the duration of hypotension. The association is more pronounced in patients undergoing longer surgeries.

Effect of peripheral nerve blocks on postanesthesia care unit length of stay in patients undergoing ambulatory surgery: a retrospective cohort study

Victor Polshin et al.

Summary: The use of peripheral nerve blocks can significantly reduce the length of stay in PACU for ambulatory surgical patients, especially in cases of longer duration and leg or ankle procedures. Patients receiving nerve blocks also require significantly lower intraoperative opioid doses.

Optimal Sedation in Patients Who Receive Neuromuscular Blocking Agent Infusions for Treatment of Acute Respiratory Distress Syndrome-A Retrospective Cohort Study From a New England Health Care Network*

Karuna Wongtangman et al.

Summary: This study showed that in patients with acute respiratory distress syndrome who receive neuromuscular blocking agent infusions, a prolonged and high proportion of deeper sedation is associated with increased mortality. Clinicians should minimize the duration of deeper sedation after recovery from neuromuscular blocking agent infusion to improve patient outcomes.

Provider variability in the intraoperative use of neuromuscular blocking agents: a retrospective multicentre cohort study

Friederike C. Althoff et al.

Summary: The study found wide variability across individual surgeons and anaesthesia providers and institutions in the use of non-depolarising neuromuscular blocking agents, which may be driven by preference and suggest that surgeons have a stronger influence on this aspect of anaesthesia management than anticipated.

Summary: The intraoperative use of high-dose nitrous oxide was found to be associated with a decrease in PACU length of stay, particularly in patients undergoing complex surgeries and receiving intraoperative antiemetic therapy.

The effect of intraoperative dexmedetomidine administration on length of stay in the post-anesthesia care unit in ambulatory surgery: A hospital registry study

Haobo Ma et al.

Summary: The intraoperative use of dexmedetomidine in ambulatory surgery is associated with prolonged PACU length of stay, which is dose-dependent and influenced by surgical duration, type of anesthesia, and timing of administration. Clinicians should carefully titrate dexmedetomidine, especially for monitored anesthesia care during shorter procedures.

Hemodynamic intolerance and pericardial effusion associated with high-frequency jet ventilation during pulmonary vein isolation

Patricia Tung et al.

Summary: A small proportion of patients undergoing pulmonary vein isolation may experience hypotension and systolic dysfunction with transient pericardial effusion during high-frequency jet ventilation (HFJV), which resolves with cessation of HFJV. The mechanism of these changes remains unclear and requires further study.
