Safety incidents and obstructive sleep apnoea in railway workers

M. Abeyaratne et al.

Summary: This study found that railway workers with severe obstructive sleep apnoea (OSA) were more likely to experience safety incidents. Furthermore, workers with severe and untreated OSA had increased odds of safety incidents. Therefore, stricter screening to identify and treat severe OSA cases should be considered to ensure railway safety.

High night-to-night variability in sleep apnea severity is associated with uncontrolled hypertension

Bastien Lechat et al.

Summary: The severity of obstructive sleep apnea (OSA) can vary significantly from night to night. However, it is unknown how this night-to-night variability in OSA severity affects cardiovascular outcomes such as hypertension. This study aimed to determine the impact of variability in OSA severity on the likelihood of hypertension. The study involved monitoring 15,526 adults over approximately 180 nights, using an under-mattress sleep sensor device and repeated blood pressure measures. The results showed that high night-to-night variability in OSA severity is a predictor of uncontrolled hypertension, independent of the severity of OSA.

How does the regulatory context influence systems thinking in work health and safety (WHS) inspectors?

Pamela Knobel et al.

Summary: Work-related incidents have significant impacts on individuals and society as a whole. However, the adoption of systems thinking in work health and safety incident investigations is limited. This study examined how WHS inspectors perceive and understand systems thinking in the regulatory context. The findings showed that inspectors often attribute systems thinking to procedures and processes, and their decision-making is influenced by the narrow focus of regulatory requirements. The implications of these findings and future research directions are discussed.

Patient navigation across the cancer care continuum: An overview of systematic reviews and emerging literature

Raymond J. J. Chan et al.

Summary: Patient navigation is an effective strategy for overcoming barriers and reducing disparities in cancer care. This umbrella review identified 61 systematic reviews and 53 primary studies, which showed that navigation improves participation in cancer screening, reduces time to diagnosis and treatment initiation, and enhances quality of life and patient satisfaction in survivorship. Economic evaluations suggest the potential cost-effectiveness of navigation in screening programs.

Results showed that low diagnostic costs, fewer follow-up visits, minimal waiting time for sleep study results, and sleep specialists recommending treatment were generally preferred by respondents.

Shift work disorder and the prevalence of help seeking behaviors for sleep concerns in Australia: A descriptive study

Brandon W. J. Brown et al.

Summary: Only 10.5% of Australian shift workers met the criteria for probable shift work disorder (pSWD), and they did not seek help for sleep problems at higher rates. Self-management strategies, such as alcohol use and caffeine consumption, were common among workers with pSWD. The mentality of 'accept it and keep going' may serve as a barrier to help-seeking.

Assessment, referral and management of obstructive sleep apnea by Australian general practitioners: a qualitative analysis

Nicole Grivell et al.

Summary: The study found that Australian general practitioners play an important role in recognizing OSA symptoms and facilitating diagnosis by others, but currently have limited involvement in OSA management. Some general practitioners, especially those working with patients in rural/remote areas and socio-economically disadvantaged areas, showed interest in being more involved in OSA management. Inequities in access to assessment and management may drive changes in future models of care for OSA in general practice.

Primary care management of chronic insomnia: a qualitative analysis of the attitudes and experiences of Australian general practitioners

Jenn Haycock et al.

Summary: This study explored the attitudes and opinions of Australian general practitioners regarding insomnia management, revealing challenges such as complexities in treatment, navigating treatment pathways, and the responsibility for care. General practitioners accepted responsibility for managing insomnia, but faced difficulties in providing care within funding and time constraints, particularly when patients presented with comorbid mental health conditions and insomnia, or when making decisions about long-term benzodiazepine use. Improved funding options, education on effective treatments, and referral pathways to specialists would benefit insomnia management within general practice.

The results suggest that the economic costs associated with sleep disorders are substantial, with the financial component equivalent to 0.73% of Australian gross domestic product.

Summary: The study estimated the economic cost of common sleep disorders in Australia for 2019-2020, totaling $35.4 billion, with obstructive sleep apnea, insomnia, and restless legs syndrome being the main contributors. Financial costs accounted for $10.0 billion, equivalent to 0.73% of Australian GDP, while the nonfinancial cost was $25.4 billion, representing 3.2% of total Australian burden of disease. The study suggests greater expenditure on detection, treatment, and prevention of sleep disorders is warranted.
