The Gum-Gut Axis: Periodontitis and the Risk of Gastrointestinal Cancers

Giacomo Baima et al.

Summary: Periodontitis and specific oral bacteria have been associated with an increased risk of gastrointestinal cancers, possibly through the translocation of oral bacteria to the gastrointestinal tract and the production of carcinogenic metabolites, immune evasion, and systemic inflammation. These findings have important implications for oral health and gastrointestinal cancer prevention.

Periodontitis and Accelerated Biological Aging: A Geroscience Approach

G. Baima et al.

Summary: As the global population ages, periodontitis and tooth loss are of great concern for health. Recent advances in geroscience have shown that biological aging is linked to periodontitis, with individuals having accelerated aging experiencing higher risks of periodontitis and tooth loss. Furthermore, cellular and molecular mechanisms at the periodontal level play a role in the aging process, with cellular senescence, stem cell exhaustion, and immunoaging impacting periodontal homeostasis and the pathophysiology of periodontitis. Advances in senotherapy involving physical exercise, intermittent fasting, natural compounds, senolytic drugs, and cell therapy are being explored to rejuvenate oral health and potentially intercept biological aging before the onset of aging-related diseases in chronologically young individuals.

Summary: The concept of the hallmarks of cancer is a tool for simplifying the complexity of cancer phenotypes and genotypes into basic principles. This article proposes phenotypic plasticity and disrupted differentiation as distinct hallmark capabilities, and discusses nonmutational epigenetic reprogramming and polymorphic microbiomes as enabling characteristics. Additionally, the importance of senescent cells in the tumor microenvironment is highlighted. The integrative concept of the hallmarks of cancer helps in understanding the mechanisms of cancer development and applying that knowledge to cancer medicine.

Local and systemic mechanisms linking periodontal disease and inflammatory comorbidities

George Hajishengallis et al.

Summary: Periodontitis is causally linked to the development of other chronic inflammatory diseases outside the oral mucosa, and intervention studies have shown that local treatment of periodontitis can ameliorate surrogate markers of comorbid conditions. Experimental animal studies have further strengthened the potential causal link by establishing biologically plausible and clinically consistent mechanisms. Understanding the impact of disseminated periodontal pathogens and systemic inflammation associated with periodontitis on extra-oral pathologies may lead to new therapeutic options to reduce the risk of periodontitis-associated comorbidities.
