Epidemiology of HIV in the USA: epidemic burden, inequities, contexts, and responses

Summary: The HIV epidemic in the USA is now concentrated in the Southern regions and there are profound disparities across regions and by race or ethnicity. This is likely driven by restricted Medicaid expansion, healthcare provider shortages, low health literacy, and HIV stigma. Overall, HIV diagnoses decreased from 2009-2018, but increased among individuals aged 25-34 years, with stable rates among White MSM, increasing rates among Black MSM, and Hispanic or Latino MSM. Surveillance data indicates ongoing disparities in HIV cases, with disproportionate impact among people in the South, racial or ethnic minorities, and MSM.

Summary: In this study, anemia was the most predominant peripheral cytopenia among HIV infected children, strongly associated with AZT based drug type, age below 10 years and high viral load. Early investigation and management of cytopenia should be emphasized, in addition to the use of alternative drugs with higher viral suppression and lower risk of toxicity.

Summary: Multiple HIV outbreaks in Pakistan since 2003 have been linked to unsafe injection practices and injection drug use. A systematic review identified 7 outbreaks from 2000 to 2019, with more than half occurring during 2016-2019. The primary sources of transmission were iatrogenic transmission, injection drug use, and a combination of both.

Summary: The prevalence of depression among HIV/AIDS patients in Pakistan is very high at 32.2%. Factors such as living in rural areas, fear of stigma and discrimination, having worked abroad, and history of substance abuse are significantly associated with depression in this population. It is recommended that guidelines be implemented to screen and treat depression among HIV patients in Pakistan.
