Prenatal risk factors for urgent atrial septostomy at birth in fetuses with transposition of the great arteries: a systematic review and meta-analysis

Summary: This study investigates the role of fetal echocardiography in predicting the need for urgent balloon atrial septostomy (BAS) in fetuses diagnosed with transposition of the great arteries (TGA) at birth. Specific echocardiographic parameters were found to be associated with the need for BAS, such as the restrictive appearance of the patent foramen ovale (FO) and hypermobile appearance of the atrial septum. However, not all echocardiographic parameters were found to be predictive of BAS. Further studies are needed to develop accurate predictive models for identifying fetuses at higher risk of urgent BAS after birth.

These observations may provide a better understanding of premature closure of the foramen ovale and postnatal hypoxia that are specific to TGA-IVS physiology.

Urgent neonatal balloon atrial septostomy in simple transposition of the great arteries: predictive value of fetal cardiac parameters

Summary: The study investigated the impact of abnormal perinatal loading conditions on the cardiac geometry and function of term fetuses and neonates with transposition of the great arteries. It found that fetal cardiac parameters could predict the need for an urgent balloon atrial septostomy after birth in neonates with transposition of the great arteries. The findings suggest that detailed cardiac assessment of fetuses with transposition of the great arteries near term could improve perinatal management and timing of intervention strategies.

Prenatal sonography of the foramen ovale predicts urgent balloon atrial septostomy in neonates with complete transposition of the great arteries

Summary: This retrospective cohort study aimed to predict the need of an urgent balloon atrial septostomy in neonates with dextro-transposition of the great arteries using prenatal echocardiography. The study found that the size of the foramen ovale was the best predictor for determining the necessity of an urgent balloon atrial septostomy, with a measurement of >6.5 mm having a sensitivity of 100% and a false positive rate of 45%.
