Effect of the Paravertebral Block on Chronic Postsurgical Pain After Thoracic Surgery: A Systematic Review and Meta-Analysis of Randomized Controlled Trials

Hyo-Seok Na et al.

Summary: This study aimed to assess the benefits of thoracic paravertebral block (PVB) in reducing chronic postsurgical pain (CPSP) after thoracic surgery. The results showed that PVB did not significantly reduce the incidence of CPSP at both 3 months and 6 months after surgery compared to no block and other block groups. However, PVB was effective in decreasing postoperative pain at 24 and 48 hours at rest.

Prevention and Management of Chronic Postsurgical Pain and Persistent Opioid Use Following Solid Organ Transplantation: Experiences From the Toronto General Hospital Transitional Pain Service

Hai Chuan Yu et al.

Summary: In this retrospective cohort study of 140 solid organ transplant patients, treatment by the TGH Transitional Pain Service team resulted in reduced opioid consumption and significant improvement in pain severity. Therefore, robust follow-up and management by a multidisciplinary team should be considered after solid organ transplantation to prevent chronic postsurgical pain, guide opioid weaning, and provide psychological support to improve patient recovery and quality of life.

Summary: This study retrospectively examined the incidence and risk factors of chronic post-surgical pain (CPSP) following single-stage implant-based breast reconstruction (IBBR). The results identified severe acute postoperative pain, a history of preoperative chronic pain, and satisfaction with the reconstructed breasts as independent predictors of CPSP.

Postoperative opioid prescribing, use and pain trends following general surgery procedures: a retrospective cohort study among veterans comparing non-opioid versus chronic opioid users

Josh Bleicher et al.

Summary: Understanding the patterns of postoperative opioid use among different populations is important for developing opioid stewardship programs. This retrospective study investigated opioid prescribing, use, and pain after general surgery procedures for patients at a veterans administration hospital. The results showed that both non-opioid users and chronic opioid users required very few opioids after surgery, and chronic opioid users quickly returned to their baseline opioid use. There was no difference in pain recovery between the two groups. The study suggests that opioid prescribing guidelines should consider including chronic opioid therapy patients and recommend a more conservative approach.

Summary: This study found that single-port video-assisted thoracic surgery (SP-VATS) reduced the prevalence of chronic postsurgical pain (CPSP) at 3- and 6-months post-pulmonary resection compared with multi-port VATS (MP-VATS). The SP-VATS technique also significantly alleviated pain interference with the daily living activities of the patients.

Summary: This study aimed to investigate the severity of chronic postsurgical pain (CPSP) in patients undergoing off-pump coronary artery bypass grafting (OP-CABG) and the impact of preemptive erector spinae plane block (ESPB) on CPSP. The study found that CPSP intensity was lower in the ESPB group compared to the control group, and significant between-group differences were observed in other outcomes.

Prevention, diagnosis, and treatment of opioid use disorder under the supervision of opioid stewardship programs: it's time to act now

Eun-Ji Kim et al.

Summary: The third opium war may have already begun due to illicit opioid trafficking and opioid misuse, making opioid use disorder a leading cause of death in the US. Screening tools such as the Opioid Risk Tool and the Current Opioid Misuse Measure can help prevent OUD, and treatment options include medication and non-medication approaches. Implementing opioid stewardship programs is crucial for improving patient outcomes and reducing opioid misuse.

Analgesic Effect of Intraoperative Intravenous S-Ketamine in Opioid-Naive Patients After Major Lumbar Fusion Surgery Is Temporary and Not Dose-Dependent: A Randomized, Double-Blind, Placebo-Controlled Clinical Trial

Summary: This study compared the effects of different doses of S-ketamine with a placebo on postoperative analgesic consumption, pain, and adverse events in adult, opioid-naive patients after lumbar fusion surgery. The results showed that neither dose of intraoperative IV S-ketamine was superior to the placebo in reducing oxycodone consumption at 48 hours post-surgery. Ketamine treatment did lead to significantly lower pain scores at the fourth postoperative hour, but not later in the study period.
