Summary: This study evaluated the reliability of transcranial Doppler (ICPtcd) as a screening test for excluding intracranial hypertension. The results showed that ICPtcd has a high negative predictive value and can accurately exclude intracranial hypertension. It can be useful in situations where invasive methods are not available or cannot be used.

However, the number of studies is insufficient and the results are not conclusive in characterizing the influence of age and sex on CVR in moderate/severe TBI.

Summary: This study examined the relationship between patient-specific epidemiologic intracranial pressure thresholds and outcomes, finding that the mean hourly dose of ICP above individual threshold has stronger associations with mortality compared to traditional guidelines. Noninvasive evaluation of intracranial pressure in patients with traumatic brain injury by transcranial Doppler ultrasound

Summary: The study investigated the relationship between ONSD or PI and ICP in TBI patients, finding a correlation between ONSD and ICP, with a stronger correlation when ONSD >= 5 mm. There was also a strong correlation between PI and ICP, especially on days 6 and 7 after surgery. A combination of ONSD >= 5 mm and PI >= 1.2 could more accurately predict elevated ICP.

Implementation of Multimodality Neurologic Monitoring Reporting in Pediatric Traumatic Brain Injury Management

Summary: The study describes the implementation of standardized MMM reporting in a pediatric ICU, leading to clinical decision-making improvements related to neuroimaging timing, ICP monitoring and mechanical ventilator duration. It was associated with a reduction in ICP monitoring and mechanical ventilator duration, but did not significantly impact other outcomes such as initial GCS or PRISM III scores, hospitalization length, complications, or GOS-E Peds scores. Prospective research is needed to further investigate the impact of MMM on functional outcomes and clinical care efficacy.

Summary: Cerebral blood flow is regulated by cerebral vessels within a range of systemic arterial pressures through a process known as cerebral autoregulation. Traumatic brain injury can impair this autoregulation, leading to changes in the autoregulatory curve. Surrogate metrics, such as cerebrovascular reactivity (CVR) metrics, have been increasingly used in bedside TBI care to assess individualized cerebral perfusion pressure targets. While some metrics show the ability to discriminate the lower limit of autoregulation, further research is needed to evaluate their validity against the upper limit.
