Short- and Long-term Outcomes of Kidney Transplants From Very Small (≤15 kg) Pediatric Donors With Acute Kidney Injury

Summary: Acute kidney injury in small pediatric donors affects early kidney graft function, but doesn't increase the risk of early graft loss and decreases long-term function. The high nonutilization rates for en bloc kidney grafts from very small pediatric donors with AKI seem unjustified, and the reluctance to transplant single kidneys from larger pediatric donors with AKI lacks a rational basis. Further prospective studies are needed for confirmation.

DCD Renal Transplantation From Donors With Acute Kidney Injury

Summary: The study found that renal transplantation from DCD donors with AKI carries a higher risk of delayed graft function, while the overall and death-censored graft failure risks were similar to those from brain dead donors. After adjusting for covariates, recipients of renal transplants from DCD donors with stage 1 and 2 AKI had a higher risk of overall graft failure.

Safety and effectiveness of kidney transplantation using a donation after brain death donor with acute kidney injury: a retrospective cohort study

Summary: This study evaluated the impact of kidney transplantation from donors with acute kidney injury (AKI) on long-term graft survival and found that AKI in donor did not affect long-term death-censored graft survival. Among recipients receiving kidneys with AKI, using low-dose r-ATG as an induction immunosuppression may offer better graft survival compared to basiliximab.
