Histopathological Features of Parathyroid Adenoma and 18F-Choline Uptake in PET/MR of Primary Hyperparathyroidism

Summary: The aim of this study was to assess the relationship between the histopathological properties of hyperfunctioning parathyroids and parathyroid F-18-choline uptake. The findings suggest that F-18-choline uptake of parathyroid adenomas might be associated both with the histological growth pattern and adenoma volume, but not with a specific cell type.

The results indicate that positron emission tomography using radiolabeled 18F-fluorocholine may be a cost-effective imaging option in the United States for this condition. A retrospective analysis of the diagnostic performance of 11C-choline PET/CT for detection of hyperfunctioning parathyroid glands after prior negative or discordant imaging in primary hyperparathyroidism

Summary: This study investigated the diagnostic performance of C-11-choline PET/CT in patients with pHPT after prior negative or discordant imaging. The results showed that C-11-choline PET/CT had a per-lesion sensitivity of 97%, a positive predictive value of 95% and an accuracy of 94%. In patients with pHPT and prior negative or discordant first-line imaging results, C-11-choline PET/CT can accurately and sensitively localize pathological parathyroid glands.

Summary: The study demonstrates that 18F-FCH PET/MR is highly effective in preoperatively localizing parathyroid adenomas, even when 18F-FCH PET/CT is negative. Neck MR alone is insufficient in detecting parathyroid adenomas, but combining PET/MR helps in precise localization.
