A Difficult and Rare Case of Warfarin Refractory Antiphospholipid Syndrome Presenting With Catastrophic Antiphospholipid Syndrome Complicated by Gastrointestinal Bleeding

Summary: This case presents a rare occurrence of catastrophic antiphospholipid syndrome (CAPS) in a patient with a history of APS who was on therapeutic doses of warfarin and aspirin therapy. Despite the recommended treatment for APS, the patient developed CAPS which did not respond to warfarin therapy, highlighting the importance of early diagnosis and aggressive treatment interventions in managing CAPS. The patient showed significant improvement in symptoms with high-dose dexamethasone, intravenous immunoglobulin (IVIG), and plasma exchange therapy.
