Defining Patient-Level Molecular Heterogeneity in Psoriasis Vulgaris Based on Single-Cell Transcriptomics

Summary: Identifying genetic variation underlying human diseases is crucial for therapeutic development and personalized treatments. In this study, we used single-cell RNA sequencing to investigate molecular variance in patients with psoriasis, a chronic skin inflammatory disease. We found that psoriasis-specific transcriptional abnormalities have significant inter-individual variance, potentially contributing to clinical heterogeneity. We also identified a chemokine, CXCL13, that is significantly correlated with lesion severity. Additionally, our analysis revealed that genes elevated in psoriatic skin-resident memory T cells are involved in chromatin remodeling and cytoskeleton restructuring, which are correlated with Th17 identity at a single-cell level.

Despite high expression of coinhibitory receptors, the psoriatic Tc17 cells exhibited upregulated cytokine, cytolytic, and metabolic activity, distinguishing them from an exhaustion program observed in melanoma-infiltrating CD8(+) T cells.
