The BSCM score: a guideline for surgical decision-making for brainstem cavernous malformations

Yang Yang et al.

Summary: The study introduced a score system to aid clinicians in treatment decision-making for patients with BSCMs, based on factors such as patient age, lesion size, hemorrhagic events, mRS, and axial location. The score scale classified patients into different grades to determine whether surgery or non-surgery was the optimal treatment option. The proposed BSCM operating score was found to be a useful tool for neurosurgeons in determining the most suitable treatment approach for patients with BSCMs.

External validation of the Lawton brainstem cavernous malformation grading system in a cohort of 277 microsurgical patients

Joshua S. Catapano et al.

Summary: This study validates the accuracy of the Brainstem Cavernous Malformation (BSCM) grading system in predicting neurological outcomes in a large cohort of patients from two high-volume surgeons. The BSCM grading system establishes categories of low-, intermediate-, and high-grade BSCMs and provides guidance for surgical decision-making.

Assessment and validation of proposed classification tools for brainstem cavernous malformations

Alejandro N. Santos et al.

Summary: Treatment indications for patients with brainstem cavernous malformations (BSCMs) are difficult and controversial. Authors aimed to validate the performance of two proposed BSCM grading systems. The study found that multiple bleedings, lesion diameter, and patient age were predictors of unfavorable postoperative outcomes, and that both the LG and the DS scores were robust tools for estimating patient outcome.

Cerebral cavernous malformation remnants after surgery: a single-center series with long-term bleeding risk analysis

Marco M. Fontanella et al.

Summary: This study evaluated clinical and radiological factors influencing the bleeding risk of a CCM post-surgical remnant and found that the presence of pre-operative perilesional hemosiderin ring and Zabramski Type II strongly condition the bleeding risk of a CCM post-surgical remnant.

The benefit and outcome prediction of acute surgery for hemorrhagic brainstem cavernous malformation with impending respiratory failure

Zixiao Yang et al.

Summary: Impending respiratory failure caused by repeated hemorrhagic brainstem cavernous malformation can benefit from acute surgical treatment. The ICH score, PPH score, and Lawton's BSCM grading are promising tools for predicting surgical outcomes efficiently.

Cavernous Malformation Surgery in the United States: Validation of a Novel International Classification of Disease, 10th Edition, Clinical Modification Code Search Algorithm and Volume-Driven Surgical Outcomes

Kavelin Rumalla et al.

Summary: The study internally validated an ICD-10 coding algorithm for identifying CM surgery accurately. Patient outcomes were influenced by factors such as age, clinical presentation, CM location, and institutional experience. Centralization of care in specialized centers may lead to improved outcomes and should be further investigated.

A comparative analysis of the modified Rankin Scale, Karnofsky Performance Status and Kurtzke expanded disability status scale in the perioperative management of patients with brainstem cavernous malformations

Yukihiro Goto et al.

Summary: In this study, the usefulness of the Kurtzke Expanded Disability Status Scale (EDSS) for assessing brainstem cerebral cavernous malformation (BCM) patients was investigated. The results showed that EDSS allowed for a more accurate assessment of patient status compared to Karnofsky Performance Status (KPS) or modified Rankin scale (mRS), particularly during the perioperative period. This suggests that EDSS may serve as a superior alternative scale for managing BCM patients.

Natural history of incidentally diagnosed brainstem cavernous malformations in a prospective observational cohort

Jing-Jie Zheng et al.

Summary: This study evaluated the hemorrhage rate and neurological outcomes of incidental brainstem cavernous malformations. Important independent risk factors included age, lesion size, developmental venous anomaly, and perilesional edema. Most patients showed improved or stable neurological function.
