Immune Reconstitution After Allogeneic Haematopoietic Cell Transplantation: From Observational Studies to Targeted Interventions

Asaf Yanir et al.

Summary: Immune reconstitution after allogeneic hematopoietic cell transplantation is crucial for transplant outcomes. Younger children experience faster immune cell recovery, leading to lower rates of graft vs. host disease and complications. Strategies to influence immune reconstitution are still evolving. Recovery of antigen-specific immune cells plays a significant role in protecting against viral diseases, while the association between immune reconstitution and graft vs. leukemia effect is less understood. Future improvements in immune reconstitution may impact transplant procedures.

Post-transplant cyclophosphamide alters immune signatures and leads to impaired T cell reconstitution in allogeneic hematopoietic stem cell transplant

Chenchen Zhao et al.

Summary: This study investigates the impact of post-transplant cyclophosphamide (PTCy) on immune reconstitution post-allogeneic hematopoietic stem cell transplantation (allo-HSCT), with a focus on T cells. The results show that PTCy significantly delays T cell reconstitution and affects T cell subsets, increasing regulatory T cells (Treg) and reducing naive T cells. It also leads to the upregulation of inhibitory receptors on CD4(+) and CD8(+) T cells, resulting in less functional CD8 T cells. The findings provide critical information for understanding the mechanism of PTCy's effect on immune reconstitution and optimizing clinical practices.

Impaired T- and NK-cell reconstitution after haploidentical HCT with posttransplant cyclophosphamide

Benedetta Rambaldi et al.

Summary: Administration of posttransplant cyclophosphamide has increased the number of patients undergoing HLA-haploidentical hematopoietic cell transplantation. Immune reconstitution in haplo-HCT patients showed delayed T-cell recovery skewed towards effector memory T cells and higher regulatory T cell ratios early after transplant. NK cell reconstitution also differed, with increased immature NK cells and enrichment of specific NK cell metadusters after haplo-HCT.

Insights from integrating clinical and preclinical studies advance understanding of graft-versus-host disease

Gerard Socie et al.

Summary: In the past 15 years, understanding of the pathophysiologic mechanisms underlying GVHD has significantly improved, but translation to clinical care has been slow and the clinical utility of new therapeutic agents has been inconsistent. New tools in immunology have recently been applied in the field of clinical GVHD, but there is a relative paucity of mechanistic insights into human translational research.
