Successful Explantation of Berlin Heart Excor in Two Young Children

Summary: Berlin Heart EXCOR ventricular assist device (VAD) implantation is commonly used as a bridge to heart transplantation in children. The use of Berlin left ventricular assist device (LVAD) as a bridge to recovery is rare. There is a lack of literature on systematic evaluation of pediatric VAD explantation. Two cases of young infants with severe heart failure were successfully treated with Berlin LVAD implantation and later underwent successful explantation after evidence of myocardial recovery.

Pediatric heart transplant waiting times in the United States since the 2016 allocation policy change

Summary: This study described waiting times for pediatric heart transplant candidates after the 2016 revision of the US allocation policy and found substantial differences in median wait times based on listing status, blood type, and recipient weight. Candidates under 25kg and with blood type O experienced lower likelihood of heart transplant and longer wait times with higher wait list mortality.

Clinical myocardial recovery in advanced heart failure with long term left ventricular assist device support

Summary: Left ventricular assist-device (LVAD) implantation is a life-saving therapy for patients with advanced heart failure. However, the translation of reverse remodeling and cardiac recovery after LVAD support into meaningful outcomes, such as explantation, is challenging. In selected patients, significant functional improvement and long-term freedom from recurrent heart failure events have been demonstrated. Predictors of myocardial recovery suggest the potential use of less invasive VADs in early-stage heart failure. It is important to develop tools to predict HF reversibility, optimize recovery with medical therapy, and monitor myocardial improvement before LVAD implantation.

Ventricular assist device support for children with chemotherapy-induced cardiomyopathy and advanced heart failure: Perspectives gained from a single-center experience

Summary: This study retrospectively analyzed the use of ventricular assist devices (VADs) in children with chemotherapy-induced cardiomyopathy. The results showed that durable VAD support can be considered as a treatment option for advanced heart failure in these children, even within 2 years of completing cancer treatment. Comprehensive assessment prior to VAD implantation and comprehensive care throughout the duration of VAD support are essential.
