Voxel-wise Functional Connectivity of the Default Mode Network in Epilepsies: A Systematic Review and Meta-Analysis

Summary: Through a systematic review and meta-analysis of 22 articles, this study found both common and specific alterations of functional connectivity (FC) in the default mode network (DMN) across different types of epilepsy. These findings contribute to our understanding of epileptic activities and cognitive behaviors, and support DMN as a potential target for effective intervention in epilepsy.

Amplitude of Low-Frequency Fluctuation With Different Clinical Outcomes in Patients With Generalized Tonic-Clonic Seizures

Summary: The study aimed to explore the neural substrate differences in the response to antiseizure drugs between seizure-free (SF) and non-seizure-free (NSF) patients with generalized tonic-clonic seizures (GTCS). Through the analysis of amplitude of low-frequency fluctuation (ALFF) in resting-state functional magnetic resonance imaging (rs-fMRI) data, it was found that the NSF group exhibited higher abnormal brain activity in the fusiform gyrus (FG) compared to the SF and healthy control groups. These findings suggest that abnormal brain activity in the FG may play a role in the failure of seizure control in patients with GTCS.

A Computational Biomarker of Photosensitive Epilepsy from Interictal EEG

Summary: This study aimed to test whether hyperexcitability could be inferred from resting interictal EEG data without stimulation, in order to increase understanding of epilepsy and develop new treatment methods. The findings revealed that occipital hyperexcitability was significantly higher in the photoparoxysmal response group, indicating that information about epilepsy hyperexcitability can still be obtained from EEG data without stimulation.

Connective profiles and antagonism between dynamic and static connectivity underlying generalized epilepsy

Summary: This study characterized the connective profiles and coupling relationship between dynamic and static functional connectivity in large-scale brain networks in patients with generalized epilepsy. The findings showed hypervariability and enhancement of functional connectivity, notably decreased structural connectivity in subcortical networks, as well as increased dynamic functional connectivity in default mode network of GE patients. Additionally, decreased antagonism between dFC and sFC was observed in core epileptic networks of GE patients compared to healthy controls.

How Alpha Rhythm Spatiotemporally Acts Upon the Thalamus-Default Mode Circuit in Idiopathic Generalized Epilepsy

Summary: This study investigated the spatiotemporal regulation of alpha oscillations in 45 IGE patients, showing increased temporal covariation between alpha power and BOLD fluctuations in thalamus and distributed cortical regions in epilepsy. High alpha power enhanced dynamic functional connectivity strength in thalamus, caudate, and some default mode network regions in epilepsy. The findings suggest a link between alpha rhythm and altered temporal dynamics, as well as hyperconnectivity in thalamus-default mode circuit, which may have clinical implications for seizure prediction and interventions.
