Polymer-based drug delivery systems under investigation for enzyme replacement and other therapies of lysosomal storage disorders

Summary: Lysosomes are important in maintaining cellular homeostasis, and their dysfunction is associated with various diseases, particularly lysosomal storage disorders (LSDs) caused by genetic mutations. Current treatment strategies, such as enzyme replacement therapy, face challenges including poor biodistribution, generation of blocking antibodies, and adverse effects. This article discusses the potential of drug delivery systems, specifically polymeric constructs and nanoparticles, to overcome these obstacles in LSD treatment. The formulations being tested, target diseases, and results from in vitro and in vivo studies are overviewed, along with the advantages, disadvantages, knowledge gaps, and considerations for clinical translation. Overall, polymeric nanoconstructs show promise in advancing LSD treatment.

Blood-brain barrier delivery for lysosomal storage disorders with IgG-lysosomal enzyme fusion proteins

Summary: The majority of lysosomal storage diseases affect the brain. The current intravenous enzyme replacement therapy is not effective because the recombinant lysosomal enzymes cannot cross the blood-brain barrier (BBB). However, by re-engineering biologic drugs into IgG-enzyme fusion proteins, it is possible to deliver the lysosomal enzymes to the brain through the BBB. This approach utilizes the IgG domain of the fusion protein, which acts as a molecular Trojan horse to transport the lysosomal enzymes across the BBB.
