Prepandemic viral community-acquired pneumonia: Diagnostic sensitivity and specificity of nasopharyngeal swabs and performance of clinical severity scores

Judith Berastegui-Cabrera et al.

Summary: The study aimed to assess the sensitivity and specificity of nasopharyngeal swabs for viral CAP diagnosis in adults, as well as the performance of PSI and CURB-65 severity scores. The results showed that nasopharyngeal swabs had lower sensitivity but higher specificity compared to sputum in diagnosing viral CAP. PSI and CURB-65 scores were not able to predict mortality in patients with viral CAP.

Presepsin as a diagnostic and prognostic biomarker of severe bacterial infections and COVID-19

Evdoxia Kyriazopoulou et al.

Summary: The study aimed to develop presepsin as a diagnostic marker for severe infections of bacterial and viral origin. Three cohorts were recruited: a derivation cohort from hospitalized patients, a first validation cohort from emergency department admissions, and a second validation cohort from COVID-19 pneumonia patients. Presepsin levels were measured in plasma using the PATHFAST assay. Concentrations above 350 pg/ml showed high sensitivity for sepsis diagnosis and 28-day mortality prognosis in both the derivation and validation cohorts. Presepsin may serve as a universal biomarker for severe infections and prediction of poor outcomes.

Negative predictive value of procalcitonin to rule out bacterial respiratory co-infection in critical covid-19 patients

Raquel Carbonell et al.

Summary: Procalcitonin (PCT) and C-Reactive Protein (CRP) have poor ability to predict bacterial respiratory coinfection in COVID-19 patients. However, baseline PCT values <0.3ng/mL can be used to rule out bacterial coinfection, while PCT >= 0.50ng/mL may predict worsening outcomes.

Lower serum alpha 1 antitrypsin levels in patients with severe COVID-19 compared with patients hospitalized due to non-COVID-19 pneumonia

Barak Pertzov et al.

Summary: This study found that patients admitted due to severe COVID-19 had lower levels of A1AT compared to patients admitted due to non-COVID pneumonia. Multivariate analysis showed that diabetes, older age, and high A1AT levels were all associated with increased mortality in COVID-19 patients.

The Microbial Etiology of Community-Acquired Pneumonia in Adults: from Classical Bacteriology to Host Transcriptional Signatures

Naomi J. Gadsby et al.

Summary: Pneumonia remains a common infection with high morbidity and mortality, despite modern advances. The understanding of its etiology has evolved with new techniques, identifying both known and emerging organisms. Bacteriologic techniques have identified Streptococcus pneumoniae as the most common cause, but more modern studies emphasize other bacteria and normal respiratory flora. Viral pneumonia has been shown to be important, and host transcriptional signatures may help distinguish between viral and bacterial pneumonia.

A Potential Role of Interleukin 10 in COVID-19 Pathogenesis

Ligong Lu et al.

Summary: One unique feature of the cytokine storm in COVID-19 is the significant elevation of IL-10, which was initially believed to be a negative feedback mechanism to suppress inflammation. However, clinical evidence suggests that early proinflammatory IL-10 elevation may actually play a pathological role in the severity of COVID-19.

Pneumonia

Antoni Torres et al.

Summary: Pneumonia is a common acute respiratory infection that affects individuals of all ages globally, causing high morbidity and mortality. It can be caused by various microorganisms, and timely identification of causative pathogens is crucial for effective treatment. New therapies and diagnostic tests are being developed to improve the management of pneumonia.
