Perspectives on Heyde's Syndrome and Calcific Aortic Valve Disease

Summary: Heyde's syndrome is a multisystem disorder characterized by the association between calcified aortic stenosis, gastrointestinal tract bleeding from arteriovenous malformations, and acquired von Willebrand syndrome. The prevalence and management of Heyde's syndrome remain controversial. Our review provides a unique case to highlight the diagnosis and management of Heyde's syndrome, as well as briefly describing the prevalence, etiology, diagnosis, and management of calcific aortic valve disease in general.

Reduction of Gastrointestinal Bleeding in Patients With Heyde Syndrome Undergoing Transcatheter Aortic Valve Implantation

Lia C. M. J. Goltstein et al.

Summary: This study found that gastrointestinal bleeding decreases in Heyde syndrome patients after TAVI, with the majority of patients experiencing relief from bleeding. Paravalvular leakage is associated with an increased risk of rebleeding, while periprocedural bleeding is more common in Heyde patients compared to non-Heyde patients.

Systemic bevacizumab as salvage therapy for persistent severe bleeding and anemia in heyde syndrome following aortic valve replacement

Zain M. Virk et al.

Summary: Heyde syndrome is a rare condition characterized by the co-occurrence of aortic stenosis and bleeding gastrointestinal angiodysplasias. This study describes two cases in which patients with persistent Heyde syndrome were successfully treated with systemic bevacizumab, leading to resolution of bleeding and normalization of hemoglobin levels. These findings suggest that targeting angiogenesis may be an effective therapeutic option for Heyde syndrome.

Summary: This article reports a case of atypical gastric mucosal bleeding caused by Heyde's syndrome. Cardiologists and gastroenterologists need to consider the possibility of Heyde's syndrome in AS patients with gastrointestinal bleeding on endoscopy.

Heyde Syndrome-Pathophysiology and Perioperative Implications

Samhati Mondal et al.

Summary: Heyde syndrome, characterized by gastrointestinal bleeding in patients with calcific aortic valve stenosis, has a strong association with valvular replacement as a definitive treatment. AVS leads to loss of Hmw VWF, increasing the risk of GI bleeds. Aortic valve replacement is crucial for recovery, with perioperative monitoring of coagulation profiles being essential.

Heyde syndrome: prevalence and outcomes in patients undergoing transcatheter aortic valve implantation

Lara Waldschmidt et al.

Summary: Heyde syndrome, characterized by severe aortic stenosis and recurrent gastrointestinal bleeding from angiodysplasia, was diagnosed in a significant number of patients undergoing TAVI. Patients with Heyde syndrome had higher rates of recurrent GIB and possibly associated with residual paravalvular leakage. Further investigation is needed to improve treatment options and outcomes for patients with Heyde syndrome.

Aortic stenosis and Heyde's syndrome: A comprehensive review

Dennisdhilak Lourdusamy et al.

Summary: Heyde's syndrome is a rare systemic disease in older adults, characterized by aortic stenosis, angiodysplasia with bleeding, and acquired von Willebrand syndrome. There are currently no established guidelines for managing the condition, but further studies may change the approach to treatment. Increasing awareness and suspicion of the syndrome can improve diagnostic potential.

Systemic bevacizumab for refractory bleeding and transfusion-dependent anemia in Heyde syndrome

Andrew B. Song et al.

Summary: In patients with Heyde syndrome, antiangiogenic therapy may be an effective treatment for severe, refractory bleeding, facilitating subsequent aortic valve replacement. Monitoring plasma vascular endothelial growth factor levels during therapy can help guide treatment decisions.
