Diffuse reflectance spectroscopy of the spine: improved breach detection with angulated fibers

Summary: Monte Carlo simulations and optical phantom experiments were conducted to investigate the impact of fiber angle on probed volume for acute breach detection. Outward angulated fibers were found to be beneficial in acute breach scenarios, as the difference in intensity magnitude between cancellous and cortical spectra increased with the fiber angle. Proximity to cortical bone could be best detected with fibers angulated at 45 degrees, and an orthopedic surgical device with a third fiber perpendicular to the device axis could cover the full impending breach range from 0 to 90 degrees.

Combined Ultrasound and Photoacoustic Image Guidance of Spinal Pedicle Cannulation Demonstrated With Intact Ex Vivo Specimens

Summary: The study developed a combined ultrasound and photoacoustic image guidance system to assist in accurate placement of pedicle screws and avoidance of bone breaches in spinal fusion surgeries. The use of coherence-based ultrasound imaging improved visualization of bony structures and successfully distinguished signals within the pedicle from impending bone breaches. These results show promise in enhancing visualization and assisting surgeons in identifying and preventing impending bone breaches during pedicle cannulation.
