Summary: Glaucoma following pediatric cataract surgery is a serious complication. Various risk factors have been reported, but the optimal surgical approach is unclear. This review evaluates the diagnostic criteria, classification, risk factors, mechanism, and surgical management of GFCS, particularly the efficacy of minimally invasive glaucoma surgery, and proposes an optimal clinical management strategy for GFCS. The results suggest that ab interno trabeculotomy (goniotomy) may be the most appropriate first-line treatment for GFCS.

The literature suggests that glaucoma drainage device implantation provides the best long-term intraocular pressure control for childhood GFCS.

The results showed that IOP spikes were a significant risk factor for GATT failure, while postoperative hyphema was a significant risk factor for IOP spikes. The use of nonsteroidal anti-inflammatory drugs (NSAIDs) may decrease the risk of IOP spikes and failure.
