Summary: Although endometriosis is a common gynecological condition, its complications are often missed as a differential diagnosis for acute abdominal pain. However, these complications can be life-threatening and require immediate treatment, often involving surgery. Obstructive complications in the bowel or urinary tract can occur due to the mass effect of endometriotic implants, while inflammatory mediators released by the ectopic endometrial tissue can result in inflammation of surrounding tissues or superinfection of the implants. Magnetic resonance imaging is the best diagnostic tool, but computed tomography can also provide an accurate diagnosis, especially when there are suggestive lesions.

ENDO_STAGE Magnetic Resonance Imaging: Classification to Screen Endometriosis

Summary: Transvaginal sonography is commonly used for diagnosing endometriosis, while magnetic resonance imaging is more accurate in staging the extent of lesions. However, the existing classifications for staging endometriosis have weaknesses in terms of complexity, reproducibility, and correlation with surgical complications. This study proposes a new classification called Endo-Stage MRI based on patterns observed with MRI. The classification system shows promise in predicting surgical complications and may improve care pathways.

Summary: Magnetic resonance imaging (MRI) is effective for diagnosing and staging deep infiltrative endometriosis (DIE). The diagnostic performance of MRI sequences susceptible to chronic blood degradation products, such as T2*-weighted imaging, is still uncertain. This study evaluated the usefulness of these sequences in addition to the conventional protocol for DIE assessment. The results showed that T2*-weighted sequences do not provide significant added value in the evaluation of DIE, especially for less experienced readers. Additionally, artifacts caused by undesirable sources of magnetic inhomogeneity may lead to overdiagnosis.

Summary: Pelvic inflammatory disease (PID) often results from sexually transmitted pathogens, leading to subtle or asymptomatic symptoms that may cause tubal factor infertility, ectopic pregnancy, or chronic pelvic pain. Neisseria gonorrhoeae and Chlamydia trachomatis are the most common PID pathogens, surviving within the host and requiring T-cell-mediated interferon gamma production for protection. Studies have shown multiple immune evasion mechanisms of these pathogens, but similarities in host immune defenses against both bacteria instill optimism for a combined vaccine for prevention of PID and infections.
