Treatment of cardiac fibrosis: from neuro-hormonal inhibitors to CAR-T cell therapy

Summary: Cardiac fibrosis, characterized by the deposition of extracellular matrix proteins, is a major factor in the development of cardiovascular disorders like heart failure. Fibroblasts play a crucial role in this process. Inhibitors of the renin-angiotensin-aldosterone system have shown some potential in reducing cardiac fibrosis. A new approach involving the use of engineered CAR-T cells has shown promise in reducing fibrosis and improving cardiac function in mouse models. Clinical studies are needed to further evaluate this approach in humans.

Summary: Myocardial fibrosis, characterized by the expansion of cardiac interstitium through deposition of extracellular matrix proteins, is a common pathophysiologic feature in various myocardial conditions. Activated fibroblasts and myofibroblasts play a central role in cardiac fibrosis, producing matrix proteins and triggering fibrogenic signalling cascades in response to stress. Immune cells, vascular cells, and cardiomyocytes can also contribute to fibrosis, while fibrotic changes may disrupt cardiac function and play a role in arrhythmogenesis.
