Glucokinase activity in diabetes: too much of a good thing?

Summary: Type 2 diabetes is a global health problem characterized by inadequate insulin secretion and chronic high blood sugar. While it was initially believed that drugs stimulating glucokinase would enhance insulin secretion, recent studies have shown that it is the hyperactivation of glucose metabolism, not glucose itself, that leads to the progressive decline in beta-cell function. This perspective discusses the potential exacerbation of this decline by glucokinase activators and counterintuitively proposes that glucokinase inhibitors might be a better therapeutic strategy for preserving beta-cell function in Type 2 diabetes.

Adiponectin reverses β-Cell damage and impaired insulin secretion induced by obesity

Summary: Obesity decreases life expectancy and increases age-related dysfunctions, including beta-cell dysregulation. Diluted plasma from obese human donors and sera from obese rats impair beta-cell integrity and insulin secretion, while the presence of adiponectin, a protective characteristic found in lean plasma, restores beta-cell function. Low circulating adiponectin is a key damaging element for beta-cells and modulation of the adiponectin signaling pathway could be a potential therapy for preventing age-related beta-cell dysfunction.

GDF15 can enhance glucose-stimulated insulin secretion by upregulating the insulin secretion pathway, independent of GFRAL. GDF15 is incrementally elevated in patients with type 2 diabetes and positively associated with β-cell function improvement.

Chemerin as an Inducer of β Cell Proliferation Mediates Mitochondrial Homeostasis and Promotes β Cell Mass Expansion

Summary: Loss of beta cell population is a key factor in type 2 diabetes, and restoring beta cell mass through proliferation and preventing apoptosis offers a potential therapeutic approach. Chemerin, an adipokine, has been identified as a promoter of beta cell proliferation both in vivo and in vitro. Overexpression of chemerin in mice resulted in larger islet area, increased beta cell mass, improved mitochondrial homeostasis, and increased insulin synthesis.

Summary: Caloric restriction is effective in treating non-alcoholic fatty liver disease and improving insulin sensitivity, but weight loss is usually not maintained due to physiological adaptations that suppress energy expenditure. Treatment with recombinant GDF15 can reduce obesity and improve glycaemic control by suppressing food intake. Additionally, GDF15 counteracts compensatory reductions in energy expenditure, leading to greater weight loss and improvements in non-alcoholic fatty liver disease compared to caloric restriction alone. This effect of GDF15 on maintaining energy expenditure during calorie restriction requires a GFRAL-beta-adrenergic-dependent signaling axis.
