Incidence, predictors, and short-term outcomes of acute kidney injury in children with diabetic ketoacidosis: a systematic review

Summary: One-third of children with type 1 diabetes mellitus present with diabetic ketoacidosis (DKA), and many of them develop acute kidney injury (AKI) due to volume depletion and hyperglycemia-induced tubular injury. This study aimed to synthesize the existing evidence on the incidence, risk factors, and outcomes of AKI in children with DKA.

Insulin pump therapy is associated with higher rates of mild diabetic ketoacidosis compared to injection therapy: A 2-year Swedish national survey of children and adolescents with type 1 diabetes

Summary: This study compared the frequency of diabetic ketoacidosis (DKA) and metabolic derangement between insulin pump therapy and insulin injection therapy in type 1 diabetes patients. The results showed that insulin pump therapy was associated with a higher risk of mild DKA and lower HbA1c levels at hospital admission compared to insulin injection therapy.

Summary: Research findings suggest that changes in glucose-corrected sodium concentrations during diabetic ketoacidosis treatment are influenced by various factors, including sodium and chloride concentrations at presentation, pre-existing diabetes, and the sodium content of intravenous fluids. However, declines in glucose-corrected sodium concentrations are not associated with mental status changes during treatment.

Sodium-Glucose Transporter Inhibition in Adult and Pediatric Patients with Type 1 Diabetes Mellitus

Summary: SGLT inhibitors have been approved as adjunctive therapy to insulin in adults with type 1 diabetes in Europe and Japan, but similar applications have been rejected by the United States Food and Drug Administration. While these agents have shown benefits such as lower HbA1c, decreased insulin dose, and weight loss, they also come with increased risks of genital mycotic infection and DKA. Additional research is needed to minimize the risk of DKA and evaluate the cardiorenal impact of these agents in persons with type 1 diabetes.

Summary: The study compared the efficacy of insulin infusion at 0.05 Unit/kg/hour versus 0.1 Unit/kg/hour in the management of pediatric diabetic ketoacidosis (DKA), finding similar resolution times for ketoacidosis but a lower rate of complications in the low-dose group. Therefore, low-dose insulin infusion may be a safer approach in the management of pediatric DKA.
