COVID-19-Induced Myocarditis: Pathophysiological Roles of ACE2 and Toll-like Receptors

Patrizia Pannucci et al.

Summary: The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection responsible for COVID-19 commonly leads to dyspnea and fatigue, primarily affecting the lungs. However, it can also result in dysfunction of extra-pulmonary organs, particularly the cardiovascular system. Cardiac complications such as hypertension, thromboembolism, arrhythmia, and heart failure have been observed in COVID-19 patients, with myocardial injury and myocarditis being the most frequent. Understanding the mechanisms of COVID-19-induced myocarditis, including changes in ACE2 expression and immune responses, is crucial.

Diagnosis and Treatment of Acute Myocarditis A Review

Enrico Ammirati et al.

Summary: Acutemyocarditis is a sudden inflammatory injury to the myocardium, affecting around 4 to 14 people per 100 000 annually with a mortality rate of about 1% to 7%. The most common causes include viruses, autoimmune disorders, drugs, and vaccines. Symptoms include chest pain, dyspnea, and syncope. Diagnosis can be made through symptoms, biomarkers, electrocardiographic changes, and imaging/biopsy. Treatment depends on the severity, clinical presentation, and etiology.

Sex and gender differences in myocarditis and dilated cardiomyopathy: An update

DeLisa Fairweather et al.

Summary: There has been increasing interest in understanding sex and gender differences in myocarditis and dilated cardiomyopathy (DCM) in the past decade. Studies have found that myocarditis occurs more often in men, while DCM has a higher incidence in women. Men with myocarditis present with chest pain, while women tend to present with dyspnea. Toll-like receptor (TLR)2 and TLR4 signaling pathways play a central role in increasing inflammation during myocarditis and promoting remodeling and fibrosis in DCM, and these pathways are elevated in males. Further research is needed to develop disease-targeted and individualized medicine approaches based on sex and gender effects.

The results showed that only ST-segment elevation (STE) was significantly associated with LGE, and it was most frequent in lateral and inferior ECG leads. However, the usability of ECG to estimate the territorial involvement and dynamical changes of LGE in myocarditis is generally limited, and CMR should be considered for this purpose.

MiR-21 and Tocilizumab interactions improve COVID-19 myocarditis outcomes

Amir Hossein Heydari et al.

Summary: This study assessed the effects of Remdesivir (RMS) and Tocilizumab (TCZ) in COVID-19 myocarditis patients. The observational cohort study found that TCZ significantly improved patients' ejection fraction, while RMS had a better effect on improving inflammatory characteristics. However, RMS-treated patients showed exacerbated cardiac function over 7 days, and RMS had a higher mortality rate than TCZ. TCZ protects the heart by decreasing the miR-21 expression rate. In conclusion, using Tocilizumab in early diagnosed COVID-19 myocarditis patients can save their cardiac function after hospitalization and decrease the mortality rate. The miR-21 level determines the outcome and responsiveness of COVID-19 myocarditis to treatment.

Summary: Myocarditis is an inflammatory disease of the myocardium, with viral infections being the most common cause. The diagnosis of myocarditis is challenging due to its wide range of clinical manifestations, which can lead to heart failure and arrhythmias. Cardiac magnetic resonance imaging (CMR) plays a crucial role in diagnosing myocarditis. Arrhythmias, including atrial fibrillation and ventricular tachycardia, are common complications, but the mechanisms are not fully understood. Myocarditis carries a significant risk, especially in patients with extensive myocardial fibrosis. CMR can serve as a predictor of sudden cardiac death (SCD) and cardiovascular mortality.
