Epidemiology, Phenotypic and Genotypic Characterization of Carbapenem-Resistant Gram-Negative Bacteria from a Libyan Hospital

Summary: Antimicrobial resistance, especially to carbapenems, poses a major threat to public health. A study conducted in Ibn Sina Hospital in Libya identified low susceptibility to carbapenems among Gram-negative bacteria. Molecular investigations revealed the presence of New Delhi metallo-beta-lactamases-1 and Verona integron-encoded metallo-beta-lactamase enzymes in Pseudomonas spp. Additionally, a new gene and resistance gene were discovered in Acinetobacter baumannii.

Epidemicity and clonal replacement of hypervirulent carbapenem-resistant Klebsiella pneumoniae with diverse pathotypes and resistance profiles in a hospital

Summary: This study collected Klebsiella pneumoniae strains from a hospital for over two years and identified carbapenem-resistant and hypervirulent strains (CR-hvKP) using PCR. Genomic analysis revealed the sequential replacement of three clones of ST11 CR-hvKP in the hospital, with clades A and B acquiring virulence plasmids and clade C acquiring a new integrating conjugative element. Phenotypic experiments showed enhanced virulence potential of the recent epidemic clone from clade B. The emergence of enhanced virulence potential in CR-hvKP highlights the coevolution between hosts and exogenous factors and the need to monitor changes in the virulence characteristics.

Diversity in the Characteristics of Klebsiella pneumoniae ST101 of Human, Environmental, and Animal Origin

Dissemination of OXA-48-and NDM-1-Producing Enterobacterales Isolates in an Algerian Hospital

This study investigated the molecular characteristics of carbapenem-resistant Enterobacterales isolates in Eastern Algeria, revealing the prevalence of carbapenemase-encoding genes such as bla(OXA-48). Clonal expansions of K. pneumoniae and E. coli were observed, and IncM plasmids were found to be predominant. The global dissemination of OXA-48-producing Enterobacterales in Algerian hospitals and the presence of NDM-1-producing E. coli in community settings highlight the importance of investigation and control measures.

Anatomy of an extensively drug-resistant Klebsiella pneumoniae outbreak in Tuscany, Italy

This study in Lebanon revealed a high carriage rate of MDR E. coli (60.7%) and the emergence of carbapenem and colistin resistance in the Lebanese community. The high clonal diversity of MDR E. coli suggests an important dynamic of acquisition and loss of resistance while plasmid transmission was limited.
